摘要
目的探讨隆起糜烂型胃炎患者的舌苔辨证证型与幽门螺杆菌(Helicobacter pylori,Hp)感染及胃黏膜病理形态学的关系。方法对2019年1月—2022年1月就诊于上海市嘉定区中医医院中医脾胃病科128例慢性隆起糜烂型胃炎患者7种不同舌苔的中医辨证分型的Hp感染及黏膜病理形态学进行分析。结果128名慢性隆起糜烂型胃炎患者中舌苔中医证候分布为肝胃气滞证34例(26.6%)、寒邪犯胃证6例(4.7%)、胃热炽盛证28例(21.9%)、食滞胃肠证25例(19.5%)、瘀阻胃络证8例(6.3%),胃阴亏虚证6例(4.7%),脾胃虚寒证21例(16.4%),其中以肝胃气滞证最为多见。就Hp阳性率而言,不同舌苔辨证中医证型Hp的阳性率从高到低依次为胃阴亏虚证、寒邪犯胃证、胃热炽盛证、食滞胃肠证、脾胃虚寒证、瘀阻胃络证、肝胃气滞证。就胃黏膜病理形态学(胃萎缩、胃肠化生、胃异型增生)而言,7种症型均以胃窦病变为主。结论慢性隆起糜烂型胃炎患者舌苔辨证以肝胃气滞证最为多见,胃阴亏虚证中Hp的阳性率最高,7种症型均以胃窦病变为主,将中医证型与西医Hp感染以及胃黏膜病理形态学变化联系在一起,为临床实践提供一定指导。
Objective To explore the relationship between syndrome differentiation of tongue coating,Helicobacter pylori(Hp)infection and gastric mucosal pathomorphology in patients with elevated erosive gastritis.Methods A total of 128 patients with chronic protruding erosive gastritis who visited the Department of Traditional Chinese Medicine,Spleen and Stomach Diseases,Jiading District Traditional Chinese Medicine Hospital,Shanghai from January 2019 to January 2022 were included.The Hp infection and mucosal pathological morphology of the patients with 7 different tongue fur according to TCM syndrome differentiation were analyzed.Results Among the 128 patients with chronic protruding and erosive gastritis,the distribution of TCM syndromes of tongue coating was as follows:34 cases(26.6%)of liver and stomach qi stagnation syndrome,6 cases(4.7%)of cold pathogen attacking the stomach syndrome,28 cases(21.9%)of stomach heat syndrome,25 cases(19.5%)of food stagnation and stomach syndrome,8 cases(6.3%)of blood stasis syndrome,6 cases(4.7%)of stomach yin deficiency syndrome,and 21 cases(16.4%)of spleen and stomach deficiency and cold syndrome.Stomach stagnation syndrome is the most common.Conclusion The syndrome of liver-stomach qi stagnation is the most common in patients with chronic protruding and erosive gastritis,and the positive rate of Hp is the highest in the syndrome of stomach-yin deficiency.Linking TCM syndrome types with Western medicine Hp infection and gastric mucosal pathomorphological changes provides some guidance for clinical practice.
