摘要
目的探索肺脏超声(LUS)在新生儿肺复张检查和评估撤机中的临床应用价值。方法前瞻性研究2018年3月至2020年8月我中心入院诊断为新生儿肺不张的患儿共134例,包括新生儿呼吸窘迫综合征(NRDS)患儿61例、新生儿肺炎(PN)患儿36例、胎粪吸入综合征(MAS)患儿37例。①治疗中不同时间进行LUS与X线胸片检查,比较两种方法检出肺复张率的差异。②治疗中不同时间进行LUS检查,提示肺复张后即撤离呼吸机,观察LUS提示肺复张率与撤离呼吸机成功率的相关性。结果治疗中不同时间LUS与X线胸片检出肺复张率差异无统计学意义(均P>0.05)。治疗中不同时间的LUS检查提示肺复张率与撤离呼吸机成功率之间存在直线相关关系,差异有统计学意义(NRSD:γ=0.989,P<0.01;PN:γ=0.999,P<0.01;MAS:γ=0.997,P<0.01)。结论LUS可用于新生儿肺复张的检查;LUS检查提示肺复张可尝试撤离呼吸机。
Objective To explore the clinical value of pulmonary ultrasonography(LUS)in checking the normal alveoli and evaluation of ventilator withdrawal of neonates.Methods A total of 134 children admitted to our center and diagnosed with neonatal atelectasis from March 2018 to August 2020 were prospectie studied,including 61 infants with neonatal respiratory distress syndrome(NRDS),36 infants with neonatal pneumonia(PN),and 37 infants with meconium aspiration syndrome(MAS).①LUS and Chest X-ray examinations were performed at different times in the treatment,and the difference of lung reexpansion rate detected by the two methods was compared.②During the treatment,LUS examination was performed at different times,suggesting that the ventilator should be removed immediately after lung expansion,and the correlation between the lung extension rate suggested by LUS and the success rate of ventilator withdrawal was observed.Results There was no significant difference in lung reexpansion rate detected by LUS and Chest X-ray at different time during treatment(all P>0.05).LUS examination at different times during the treatment showed a linear correlation between the lung recovery rate and the success rate of ventilator withdrawal(NRSD:γ=0.989,P<0.01;PN:γ=0.999,P<0.01;MAS:γ=0.997,P<0.01).Conclusion LUS can be used for the examination of neonatal lung extension;An LUS examination suggests pulmonary revascularization and an attempt to withdraw the ventilator.
