摘要
目的:调查住院医师规范化培训期间抑郁的发展和相关因素。方法:采用PHQ-9抑郁量表,对2016-2018年参加住院医师规范化培训第一年的住院医师进行基线调查和每三个月一次跟踪随访调查。应用广义估计方程模型进行建模与分析。结果:住院医师自我评估PHQ-9抑郁症状评分从基线的4.09±4.06分至培训期间明显上升,平均达到5.98分(P<0.001)。调查对象抑郁检出率(即PHQ-9总分≥10)从基线调查的9.43%到培训期间平均达到21.34%(P<0.001)。培训类型为学位衔接、基线抑郁症状低、有个人抑郁史以及培训期间因素(工作时间增加、睡眠减少、生活中非培训相关其他压力事件和害怕遭到病人或家属的谩骂、威胁或身体伤害)是住院医师在培训过程中抑郁症状显著上升的危险因素。结论:住院医师心理健康问题亟待关注,有必要采取一定的措施以改善住院医师的抑郁状况,特别是学位衔接住院医师应予以重点关注。
Objective:To evaluate the development and related factors of depression during residents training.Methods:Depressive symptoms were measured at baseline and every 3 months of their first year of residency,using the PHQ-9 depression questionnaire.Generalized Estimated Equation model was performed to assess the association between depression change and within-training factors.Results:The self-reported PHQ-9 depression score increased from 4.09±4.06 prior to residency to a mean of 5.98(P<0.001)during the first year of residency.The proportion of participants who met PHQ-9 Criteria for depression(PHQ-9≥10)increased from 9.43%to a mean of 21.34%(P<0.001).A series of factors measured prior to residency(lower baseline depressive symptom,and history of major depression)and during residency(increased work hours,decreased sleep hours,stressful life events and fear of violence)were associated with a greater increase in depression symptoms during residency training.Conclusion:Residents’mental health needs urgent attention.It is necessary to take certain measures to improve the depression status of residents.In particular,more attention should be paid to residents who are studying for a master’s degree while conducting residency training.
