摘要
目的观察麻杏石甘汤联合阿奇霉素治疗小儿肺炎支原体肺炎的临床疗效。方法随机将医院2017年10月—2019年10月诊治100例小儿肺炎支原体肺炎患儿分为对照组和治疗组,每组各50例,对照组给予阿奇霉素进行治疗,治疗组在对照组的基础上联用麻杏石甘汤治疗,两组患儿连续治疗2周。观察两组患儿治疗前后临床疗效、临床症状改善情况、血清炎症因子水平、肺功能、支原体转阴率及不良反应情况。结果治疗后,治疗组总有效率为94.00%(47/50),对照组总有效率为78.00%(39/50),治疗组显著高于对照组(P<0.05);治疗组患儿的退热情况、气喘改善情况、咳嗽改善情况、肺部湿罗音改善情况均优于治疗前(P<0.05),且优于对照组(P<0.05);两组患儿肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、白细胞介素-17(IL-17)水平明显降低(P<0.05),治疗组低于对照组(P<0.05);两组患儿肺功能1 s用力呼气容积(FEV1值)、呼气峰值流速(PEF)值均较治疗前升高(P<0.05),治疗组高于对照组(P<0.05);治疗后,治疗组患儿支原体Ig-M转阴率高于对照组(P<0.05),对照组患儿不良反应发生率为6%,治疗组不良反应发生率为2%,差异无统计学意义(P>0.05)。结论麻杏石甘汤联合阿奇霉素治疗小儿支原体肺炎疗效显著,可提高患儿临床疗效,改善患儿临床症状,无严重不良反应,值得临床推广应用。
Objective To observe the clinical efficacy of Maxing Shigan Decoction(麻杏石甘汤) combined with azithromycin in the treatment of mycoplasma pneumonia in children. Methods A total of 100 children with mycoplasma pneumonia diagnosed and treated in our hospital from October 2017 to October 2019 were randomly divided into a control group and a treatment group, 50 in each. The control group was given azithromycin for treatment. Based on the treatment of the control group, the treatment group added Maxing Shigan Decoction. The two groups were treated continuously for 2 weeks. We observed the clinical efficacy, improvement of clinical symptoms, serum inflammatory factor levels, lung function, mycoplasma negative rate and adverse reactions in the two groups of children before and after treatment. Results After treatment, the total effective rate in the treatment group was 94.00%, and the total effective rate in the control group was 78.00%. The treatment group’s rate was significantly higher than that of the control group(P<0.05). The children in the treatment group had improved fever, asthma and cough. The condition and improvement of lung wet rales were better than those before treatment(P<0.05) and better than those of the control group(P<0.05). The levels of tumor necrosis factor(TNF)-α, interleukin-6(IL-6), IL-10 and IL-17 were significantly reduced(P<0.05), and the treatment group’s levels were lower than those of the control group(P<0.05). The pulmonary function FEV1 and PEF of the two groups were higher than those before treatment(P<0.05). The treatment group’s FEV1 and PEF were higher than those of the control group(P<0.05). After treatment, the mycoplasma IgM negative rate in the treatment group was higher than that in the control group(P<0.05), and the incidence of adverse reactions in the control group was 6%, and that in the treatment group was 2%.
