摘要
目的:探究预防失禁相关性皮炎的护理方法。方法:按照随机数字表法,将50例需灌肠治疗的重症急性胰腺炎分为实验组和对照组,各25例,因住院时间不满7天退出研究的患者分别为2例和3例,剩下的实验组23例和对照组22例,共计45例完成研究。灌肠前,实验组使用肤乐霜涂擦肛周皮肤、外生殖器及臀裂,并在臀裂覆盖水胶体敷料,对照组使用液体敷料喷洒肛周皮肤、外生殖器及臀裂。观察两组患者肛周皮肤、外生殖器及臀裂3个部位的失禁相关性皮炎皮肤损伤总得分、3个部位失禁相关性皮炎皮肤损伤各得分、日均护理费用及日均护理时间。结果:实验组失禁相关性皮炎皮肤损伤总得分、臀裂部位失禁相关性皮炎皮肤损伤得分、日均护理费用、日均护理时间均低于对照组,差异均有统计学意义(P<0.001),两组患者肛周及外生殖器部位失禁相关性皮炎皮肤损伤得分差异无统计学意义(P>0.05)。结论:肤乐霜联合水胶体敷料能降低臀裂失禁相关性皮炎的发生,节约医疗费用和护理时间。
Objective:To explore the nursing methods of preventing incontinence-associated dermatitis.Methods:A to⁃tal of 50 cases with severe acute pancreatitis who needed enema were randomly divided into experimental group(25 cas⁃es,2 patients were not included in the study later because the length of stay was less than 7 days)and control group(25 cases,3 patients were not included in the study later because the length of stay was less than 7 days),the actual cases included in the study was 45 ones(23 cases in experimental group,22 cases in control group).Before enema,the exper⁃imental group used fule cream to smear perianal skin,external genitalia and hip fissure,and covered the gluteal fissure with hydrocolloid dressings.The control group used liquid dressings to spray perianal skin,external genitalia and hip fis⁃sure.The scores(total and respective)of skin injury assessment,daily average nursing cost and daily average nursing time of incontinence-associated dermatitis(perianal skin,external genitalia and gluteal fissure)in two groups was record⁃ed.Results:The total score of incontinence-related dermatitis skin injury in the experimental group,the skin injury score of incontinence-related dermatitis at the gluteal cleft,the average daily care cost,and the average daily care time were all lower than those of the control group,and the differences were statistically significant(P<0.001).There was no signifi⁃cant difference in skin damage scores of perianal and external genital incontinence-related dermatitis between the two groups(P>0.05).Conclusion:The treatment of fule ointment combined with hydrocolloid dressings can reduce the in⁃cidence of gluteal fissure incontinence-associated dermatitis,and save medical expenses and nursing time.
