摘要
目的通过对山东省基层医疗卫生机构资源配置效率的分析,找出其存在的问题并提出相应的对策建议,为优化基层卫生资源配置提供借鉴和参考。方法通过文献分析法,对现有文献进行分析和借鉴,并结合数据可得性原则从2009—2014年《山东卫生统计年鉴》、2015—2017年《山东卫生计生统计年鉴》和2018年《山东卫生健康统计年鉴》中选取诊疗人次、入院人数2个产出指标和床位数、机构数、卫生技术人员数3个投入指标构建山东省基层医疗卫生机构资源配置效率评价指标体系,分别运用DEA方法中的BCC模型和Malmquist指数模型对资源配置效率进行静态和动态分析。结果静态效率方面,各年份的纯技术效率均值在0.850~0.913之间,总体均为技术无效状态;规模效率均值虽然都在0.9以上,但均未实现最优生产规模,各年份处于规模报酬不变状态的城市数量均不足全省城市总数的一半;综合效率值较低,但总体呈上升趋势。动态效率方面,不论是从年份还是地区来看,山东省基层医疗卫生机构的全要素生产率均没有明显提高的迹象。结论近年来山东省基层医疗卫生机构发展较快,但其资源配置效率不容乐观,要从医疗机构自身着手,通过提高组织管理水平、加强科技创新及优化人力资源结构等措施,提高基层医疗卫生资源配置的合理性和利用的充分性。
Objective To analyze the resource allocation efficiency of primary medical and health institutions in Shandong province,and to find out the existing problems and the reasons,and to put forward corresponding countermeasures and suggestions for promoting the development of primary medical and health undertakings.Methods Through literature analysis,and based on the principle of data availability,the resource allocation efficiency evaluation index system of basic medical and health institutions in Shandong province was constructed,which included 2 output indicators and 3 input indicators.The data of the indicators were extracted from Shandong Health Statistical Yearbook(2009-2014),Shandong Health and Family Planning Statistical Yearbook(2015-2017)and Shandong Health Statistical Yearbook(2018).DEA method and Malmquist index model was applied for the efficiency analysis of resource allocation.Results In terms of static efficiency,the mean value of pure technical efficiency was between 0.850-0.913 each year,which was generally invalid.Although the mean scale efficiency was above 0.9,the optimal production scale was not achieved.The number of cities in the state of constant scale return in each year was less than half of the total number of cities of the province.The comprehensive efficiency value was low,but the general trend was on the rise.In terms of dynamic efficiency,the total factor productivity of primary medical and health institutions showed no obvious improvement from the perspective of year or region.Conclusion In recent years,the primary medical and health institutions in Shandong province have developed rapidly,but their efficiency of resource allocation is not optimistic.Therefore,it is necessary to start from the medical institutions themselves and to improve the rationality and adequacy of the allocation and utilization of primary medical and health resources by improving the level of organization and management,strengthening scientific and technological innovation and optimizing the structure of human r
