摘要
目的探讨胰腺神经内分泌肿瘤(PNEN)患者的临床病理特征,为临床PNEN诊治提供参考。方法选取2015年7月—2020年7月在天津市南开医院治疗并由病理诊断确诊的12例患者,观察患者的临床表现、影像学、病理学特征等并对其预后进行随访。结果 12例患者中,男性3例,女性9例;年龄24~82岁,中位数年龄54岁;肿瘤最大径1.5~10.0 cm,均进行根治性手术治疗;根据WHO 2017版分级标准,G1级8例,G2级4例,功能性肿瘤2例,无功能性肿瘤10例,其中无功能性肿瘤多以排便困难、腹胀不适、腹痛、恶心及腹部肿块为主。镜下示肿瘤细胞呈实性,大小均一。免疫组化显示Syn阳性率为100.0%,CgA阳性率为91.6%,CD56阳性率为83.3%。本研究中9例获得随访,随访时间6~60个月,均健康且无瘤生存。结论 PNEN的临床表现较为繁杂,易造成漏诊及误诊。病理诊断及免疫组化分析是确诊PNEN的主要依据。
Objective To explore the clinicopathological features of patients with pancreatic neuroendocrine neoplasm(PNEN),so as to provide reference for clinical diagnosis and treatment. Methods A total of 12 patients with PNEN treated and diagnosed by pathology in Tianjin Nankai Hospital from July 2015 to July2020 were selected to observe the clinical manifestations,imaging and pathological characteristics of the patients and follow up their prognosis. Results Among the 12 patients with PNEN,there were 3 males and 9 females,with a median age of 54 years and a maximum diameter of the tumor was 1.5-10.0 cm,and radical surgery was performed to all. According to the grading standard of WHO 2017,there were 8 cases of G1,4 cases of G2,2 cases of functional tumor and 10 cases of non-functional tumor,among which non-functional tumors were mainly manifested by defecation difficulty,abdominal distension and discomfort,abdominal pain,nausea and abdominal mass. Microscopically,the tumor cells were solid and uniform in size. The immunohistochemistry showed that positive for Syn with 100.0%,CgA with 91.6% and CD56 with 83.3%. A total of 9 patients in this study were followed up for 6-60 months and all of them were healthy and tumor free.Conclusion The clinical manifestation of PNEN were relatively complicated,which was easy to cause missed diagnosis and misdiagnosis. Pathological diagnosis and immunohistochemical analysis were the basis for the diagnosis and prognosis of PNEN.
