摘要
目的研究小儿呼吸道感染检测过程中,C反应蛋白(CRP)、淀粉样蛋白A(SAA)、降钙素原(PCT)等指标的应用价值。方法选择180例于2019年5月至2020年12月在广州市南沙区妇幼保健院进行治疗的呼吸道感染患儿为对象,基于感染源划分,分为细菌感染组、病毒感染组,同时设置90例同条件的健康儿童为对照组。检测三组血清CRP、SAA及PCT水平并比较,将病原体检查结果作为金标准,比较三组单项、联合检测的阳性率及不同指标诊断小儿呼吸道病毒感染的效能。结果细菌感染组血清CRP、PCT水平高于对照组与病毒感染组,细菌感染组SAA指标低于病毒感染组(P<0.05)。细菌感染组CRP与SAA、PCT呈正相关(r=0.728、0.653,P<0.05)。细菌感染组血清CRP、PCT的单独和联合检测阳性率都高于其他两组,而病毒感染组均高于对照组(P<0.05)。ROC曲线分析显示,CRP+SAA、CRP+PCT诊断小儿呼吸道细菌感染的曲线下面积(AUC)均为1.000,SAA+CRP、SAA+PCT诊断小儿呼吸道病毒感染的AUC为1.000,均高于单一检测指标。结论CRP、SAA联合检测有助于提高小儿呼吸道感染的鉴别诊断效果,全身症状明显时可增加PCT检测,对患儿病情动态监测、临床用药调整具有参考性意义。
【Objective】To analyze the differential diagnosis effect of C-reactive protein(CRP),amyloid A(SAA)and procalcitonin(PCT)in children with respiratory tract infection.【Methods】From May 2019 to December 2020,90 cases of children with bacterial respiratory tract infection and 90 cases of children with viral respiratory tract infection were selected as bacterial infection group and viral infection group respectively,and another 90 cases of healthy children were selected as control group.The levels of serum CRP,SAA and PCT were detected and compared among the three groups,and the pathogen test results were taken as the gold standard.The positive rates of single and combined detection of the three groups were compared,and the efficacy of different indicators in the diagnosis of children with respiratory virus infection was compared.【Results】The levels of serum CRP and PCT in bacterial infection group were higher than those in control group and virus infection group,the levels of SAA in bacterial infection group were lower than those in virus infection group,and the difference was statistically significant(P<0.05).In bacterial infection group,CRP was positively correlated with SAA and PCT(r=0.728,0.653,P<0.05).The positive rates of CRP,PCT detection alone and combined detection in bacterial infection group were higher than those in control group and virus infection group,which in virus infection group were higher than those in control group,and the difference was statistically significant(P<0.05).ROC curve analysis showed that the AUC of CRP+SAA,CRP+PCT in the diagnosis of children with respiratory tract bacterial infection was 1.000,and the AUC of SAA+CRP,SAA+PCT in the diagnosis of children with respiratory tract virus infection was 1.000,which were higher than single detection index.【Conclusion】The combined detection of CRP and SAA is helpful to improve the differential diagnosis effect of respiratory tract infection in children.When the systemic symptoms are obvious,the detection of PCT can be increase
