摘要
目的观察膝痹汤外洗结合强骨胶囊口服对合并骨髓水肿重度膝骨关节炎患者的临床疗效。方法选取2018年9月—2020年1月期间中国中医科学院望京医院骨关节二科门诊收治的合并骨髓水肿重度膝骨关节炎患者60例,依据治疗方案分为治疗组及对照组,每组30例。治疗组采用膝痹汤外洗结合强骨胶囊口服,对照组采用洛索洛芬钠片口服,两组均治疗2周。观察两组患者治疗前后临床疗效及西安大略和麦克马斯特大学骨关节炎指数(WOMAC)、整体磁共振成像评分系统(WORMS)、视觉模拟评分法(VAS)评分、关节叩击痛程度。结果两组患者治疗后WOMAC、WORMS、VAS评分及关节叩击痛程度均较治疗前明显改善,差异有统计学意义(P<0.01),其中VAS、WORMS评分及叩击痛程度治疗组均优于对照组,差异有统计学意义(P<0.01),WOMAC评分两组治疗后比较,差异无统计学意义(P>0.05)。两组患者治疗后总有效率比较,治疗组优于对照组,差异有统计学意义(P<0.01)。结论采用膝痹汤外洗结合强骨胶囊口服对于合并骨髓水肿的重度膝骨关节炎患者疗效显著,可大幅度减轻患者疼痛,改善关节功能,安全性高。
Objective To retrospectively study the curative effect of Xibi Decoction Washing combined with Qianggu capsule oral comprehensive therapy on patients with severe knee osteoarthritis with bone marrow edema.MethodsA total of 60 patients suffering severe knee osteoarthritis with bone marrow edema at the Second Department of Orthopedics of Wangjing Hospital from September 2018 to January 2020 were included.According to the treatment plan,patients were divided into the treatment group and the control group.The treatment group has 30 cases which were treated by washing Xibi Decoction and using Qianggu capsules.The control group has 30 patients,taking loxoprofen sodium tablets.Both groups were given health education on the basis of treatment,and the course of treatment was two weeks;Western Ontario and McMaster University Osteoarthritis Index(WOMAC),Whole Magnetic Resonance Imaging Scoring System(WORMS),Visual Analogue Scale(VAS)score,degree of joint tapping were statistically analyzed before and after treatment.Results After treatment,the WOMAC,WORMS,VAS scores and the degree of joint percussion pain were significantly improved in the two groups compared with before treatment,and the difference was statistically significant(P<0.01).Among them,the VAS,WORMS scores and the degree of percussion pain in the treatment group were all improved Better than the control group,the difference was statistically significant(P<0.01),and the WOMAC score was not statistically significant between the two groups after treatment(P>0.05).The total effective rate of the two groups of patients after treatment was compared,the treatment group was better than the control group,and the difference was statistically significant(P<0.01).Conclusion Combination of external washing with Xibi Decoction and Qianggu Capsule is effective for patients with severe knee osteoarthritis complicated with bone marrow edema,which can greatly reduce the pain of patients,improve joint function,and have high safety.
