摘要
目的:通过对上海市金山区两家社区卫生服务中心2019年门诊数据分析,了解门诊就诊情况及疾病谱,为提升社区卫生服务管理水平提供借鉴。方法:通过医院信息管理系统收集门诊电子病历数据,内容包括性别、年龄、就诊时间、诊断编码、诊断名称等,根据国际疾病分类第十次修订本(ICD-10),以第一诊断进行疾病归类,并将就诊人群按性别、年龄进行分组分析。结果:通过对两社区门诊数据分析发现,男性就诊人群平均年龄、平均就诊次数均小于女性,差异具有统计学意义(P<0.001)。人均就诊次数随年龄升高而增加,差异具有统计学意义(P<0.001)。排名前3位的病种为高血压、上呼吸道感染、支气管炎;男性患者病种类数少于女性,男性和女性患者的病种类数均随着年龄的升高而增加,差异均具有统计学意义(P<0.001)。结论:金山区社区卫生服务中心的门诊患者以中老年人为主,1年多次就诊、多病共存现象普遍,人均年病种类数随着年龄的增加而增高。不同性别的患者就诊特征存在差异,两社区就诊女性患者平均年龄大于男性,平均就诊次数和病种类也高于男性。
Objective:By analyzing the outpatient data of two community health service centers in Jinshan District of Shanghai in 2019 to understand the outpatient diagnosis and disease spectrum,so as to provide reference for improving the management level of the community health service.Methods:The outpatient electronic medical record data were collected through the hospital information management system,including gender,age,time of visit,diagnostic code,diagnostic name,etc,and according to the tenth revision of the international classification of diseases(ICD-10),the first diagnosis was used to classify the diseases,and the patients were divided into groups and analyzed by gender and age.Results:By analyzing the outpatient data of the two communities,it was found that the average age and the average number of visits of men were less than that of women,and the difference was statistically significant(P<0.001).The number of visits per capita increased with age,and the difference was statistically significant(P<0.001).The top three diseases were hypertension,upper respiratory tract infection and bronchitis;the number of disease types in male patients was less than that in female patients,and the number of disease types in male and female patients increased with age,and the difference was statistically significant(P<0.001).Conclusion:The outpatients of Jinshan community health service centers are mainly middle-aged and elderly people,the phenomenon of multiple visits in one year and the coexistence of multiple diseases is common and the number of annual disease types per capita increases with the increase of age.There are differences in the treatment characteristics of patients of different genders,the average age of female patients in the two communities is older than that of male patients,and the average number of visits and types of diseases of female patients are also higher than those of male patients.
