摘要
目的对快速康复小组在髋关节置换患者围术期中的护理进行回顾性分析,以期建立更优化的护理流程。方法采用历史对照研究方法,以2019年7月开始快速康复护理为界,将2019年7月至2020年4月在吴川市人民医院骨科接受髋关节置换术患者52例设为快速康复组,2018年9月至2019年6月接受同种手术的50例患者设为常规护理组。快速康复组男36例,女16例,年龄(74.41±3.77)岁;常规护理组男35例,女15例,年龄(74.37±3.18)岁。快速康复组患者围术期实施快速康复护理流程,即强调术前宣教、个性化疼痛控制以确保患者舒适,尽早进食、下床活动、拔除管道以预防并发症,做好心理护理、功能锻炼和出院指导以优化患者康复护理指标;常规护理组患者围术期实施常规的护理流程。术后1周,比较两组患者的舒适度、并发症发生率以及生活自理能力(Barthel指数)。结果快速康复组患者舒适度的4个维度得分均较常规护理组明显提高,总分明显高于常规护理组[(106.75±3.18)分比(81.34±4.86)分],差异均有统计学意义(均P<0.001);快速康复组术后并发症发生率明显低于常规护理组[3.85%(2/52)比18.00%(9/50)],差异有统计学意义(χ^(2)=5.307,P=0.021);快速康复组患者的Barthel指数明显高于常规护理组[(66.35±3.78)分比(49.04±4.30)分],差异有统计学意义(t=21.090,P<0.001)。结论快速康复小组在髋关节置换患者围术期护理中,遵循快速康复理念,整合团队的共同力量,实施快速康复护理流程后可确保围术期患者舒适、减少术后并发症,有利于患者自理能力恢复。
Objective To retrospectively analyze the perioperative nursing of rapid rehabilitation team in patients with hip replacement,in order to establish a more optimized nursing process.Methods Using the historical control study,52 patients who underwent hip replacement in the Department of Orthopedics of People's Hospital of Wuchuan City from July 2019 to April 2020 were enrolled as a rapid rehabilitation group,and 50 patients who underwent the same operation from September 2018 to June 2019 were enrolled as a routine nursing group.There were 36 males and 16 females in the rapid rehabilitation group,with an age of(74.41±3.77)years old;in the routine nursing group,there were 35 males and 15 females,with an age of(74.37±3.18)years old.The patients in the rapid rehabilitation group were given rapid rehabilitation nursing process during the perioperative period,namely emphasizing the preoperative education and personalized pain control to ensure patients'comfort,feed,off-bed activities,and removing the tube as soon as possible to prevent complications,and doing well psychological nursing,functional exercise,and discharge guidance to optimize patients'rehabilitation nursing indicators.The patients in the routine nursing group were given routine nursing process during the perioperative period.One week after the surgery,the comfort levels,complication rates,and Barthel indexes of the 2 groups were compared.Results The scores of four dimensions of comfort level in the rapid rehabilitation group were significantly higher than those in the routine nursing group,and the total score was significantly higher than that in the routine nursing group[(106.75±3.18)points vs.(81.34±4.86)points],with statistically significant differences(all P<0.001).The incidence of postoperative complications in the rapid rehabilitation group was significantly lower than that in the routine nursing group[3.85%(2/52)vs.18.00%(9/50)],with a statistically significant difference(χ^(2)=5.307,P=0.021).The Barthel index of the patients in the rapid rehab
