摘要
目的 探究早期乳腺癌患者应用保乳联合前哨淋巴结活检术治疗的临床疗效。方法 80例早期乳腺癌患者,随机分为观察组和对照组,每组40例。观察组患者采用保乳联合前哨淋巴结活检术治疗,对照组患者行改良根治术治疗。对比两组患者手术指标(手术时间、术中出血量、总引流量、胸壁引流管拔出时间)、乳房美容效果及并发症发生情况。结果 观察组患者手术时间为(105.45±9.87)min、术中出血量为(275.12±16.75)ml、总引流量为(100.45±19.35)ml、胸壁引流管拔出时间为(5.45±1.12)d、住院时间为(10.25±1.89)d;对照组患者手术时间为(128.55±12.08)min、术中出血量为(335.45±23.25)ml、总引流量为(270.35±17.05)ml、胸壁引流管拔出时间为(7.56±2.58)d、住院时间为(15.26±2.35)d。观察组患者手术时间、胸壁引流管拔出时间、住院时间均短于对照组,术中出血量、总引流量均少于对照组,差异有统计学意义(P<0.05)。观察组患者乳房美容满意率为92.5%,高于对照组的75.0%,差异有统计学意义(P<0.05)。观察组患者并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论 采用保乳联合前哨淋巴结活检术治疗早期乳腺癌,其手术用时明显更短,术中出血少,乳房美容效果更显著,并发症发生率更低,值得临床大力推广及应用。
Objective To investigate the clinical efficacy of breast conserving surgery combined with sentinel lymph node biopsy in the treatment of early breast cancer.Methods A total of 80 patients with early breast cancer were randomly divided into observation group and control group,with 40 cases in each group.The observation group was treated with breast conserving surgery and sentinel lymph node biopsy,and the control group was treated with modified radical mastectomy.Patients in both groups were compared in terms of surgical indicators(operation time,intraoperative blood loss,total drainage volume,chest drainage tube withdrawal time),breast cosmetic effect and complications.Results In the observation group,the operation time was(105.45±9.87)min,intraoperative blood loss was(275.12±16.75)ml,total drainage volume was(100.45±19.35)ml,chest drainage tube withdrawal time was(5.45±1.12)d,hospitalization time was(10.25±1.89)d;in the control group,the operation time was(128.55±12.08)min,intraoperative blood loss was(335.45±23.25)ml,total drainage volume was(270.35±17.05),chest drainage tube withdrawal time was(7.56±2.58)d,hospitalization time was(15.26±2.35)d.The operation time,chest drainage tube withdrawal time,and hospitalization time of the observation group were shorter than those of the control group,and the intraoperative blood loss and total drainage were less than those of the control group.All the differences were statistically significant(P<0.05).The satisfaction rate with breast cosmetic effect in the observation group was 92.5%,which was higher than 75.0%in the control group,and the difference was statistically significant(P<0.05).The complication rates in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Combination of breast conserving surgery and sentinel lymph node biopsy for patients with early breast cancer has shorter operation time,less intraoperative blood loss,more significant breast cosmetic effect and lower comp
