摘要
目的探讨保乳术联合前哨淋巴结活检在老年乳腺癌患者中的效果及对患者术后恢复及生存质量的影响。方法选取2019年3月—2021年12月于福州市妇幼保健院就诊的80例老年乳腺癌患者,根据随机数字表法分为对照组40例和观察组40例。对照组行常规乳腺癌改良根治术,观察组则行保乳术联合前哨淋巴结活检。比较两组患者的手术时间、术中出血量、拔管时间、并发症发生率、住院时间、美观满意度、手术前后的肩关节活动度及生存质量(the WHO quality of life brief scale,WHOQOL-BREF量表)。结果观察组的手术时间、拔管时间及住院时间显著短于对照组,术中出血量显著少于对照组,并发症发生率显著低于对照组,美观满意度显著高于对照组(P<0.05)。术前两组的肩关节活动度及WHOQOL-BREF量表评分比较,差异无统计学意义(P>0.05);术后3个月及6个月观察组的肩关节活动度及WHOQOL-BREF量表评分显著优于对照组(P<0.05)。结论保乳术联合前哨淋巴结活检更有助于老年乳腺癌患者的术后恢复,且对生存质量的影响更为积极,因此在老年乳腺癌患者中的应用价值相对更高。
Objective To investigate the effect of breast conserving surgery combined with sentinel lymph node biopsy in elderly patients with breast cancer and influence for postoperative recovery and life quality.Methods A total of 80 elderly patients with breast cancer from March 2019 to December 2021 of Fuzhou Maternal and Child Health Hospital were divided into control group 40 cases and observation group 40 cases by the method of random number table.The control group were treated with routine radical mastectomy,the observation group were treated with breast conserving surgery combined with sentinel lymph node biopsy.Then the operation time,intraoperative blood loss,extubation time,complication rates,hospitalization time,aesthetic satisfaction,motion range of shoulder joint and life quality(WHOQOL-BREF scale)before and after the operation of two groups were compared.Results The operation time,extubation time and hospitalization time of observation group were significantly shorter than those of control group,the intraoperative blood loss was significantly less than that of control group,the complication rate was significantly lower than that of control group,the aesthetic satisfaction was significantly higher than that of control group(P<0.05),The motion range of shoulder joint and WHOQOL-BREF scale of two groups before the operation were compared,the differences were not statistically significant(P>0.05),the motion range of shoulder joint and WHOQOL-BREF scale of observation group at third and sixth month after the operation were significantly better than those of control group(P<0.05).Conclusion The breast conserving surgery combined with sentinel lymph node biopsy is more helpful to postoperative recovery of elderly patients with breast cancer,and it has better influence for the life quality,so its application value in the elderly patients with breast cancer is relatively higher.
