摘要
目的研究M型超声心动图技术评估妊娠期糖尿病胎儿和出生后新生儿心功能的价值。方法采用回顾性分析方法,以2019年6月至2020年6月自贡市第一人民医院收治的50例妊娠期糖尿病孕妇为研究对象,设定为研究组;同时,选取同期入院的50例正常单胎妊娠孕妇为研究对象,设定为对照组;2组均行M型超声心动图检查。比较2组胎儿超声心动图指标水平[左室心肌重量(LVM)、左室缩短分数(LVFS)、左室射血分数(LVEF)、二尖瓣环位移(MAD)、三尖瓣环位移(TAD)、动脉导管(AD)、卵圆孔(OF)、肺动脉内径(PAID)、主动脉内径(AID)、左心室后壁(LVPW)、室间隔(IVS)、右心房内径(RAID)、右心室内径(RVID)、左心房内径(LAID)、左心室内径(LVID)、胸横径(Th D)、心脏横径(TCD)]与出生后1周新生儿超声心动图指标水平[二尖瓣环位移(MAD)、三尖瓣环位移(TAD)、射血分数(EF)、肺动脉内径(PAID)、主动脉内径(AID)、左心室后壁(LVPW)、室间隔(IVS)、右心房内径(RAID)、右心室内径(RVID)、左心房内径(LAID)、左心室内径(LVID)]。结果 2组胎儿LVFS、LVEF、AD、OF、PAID、AID、RAID、RVID、LAID、LVID、Th D、TCD比较,差异无统计学意义(P>0.05);研究组胎儿较对照组LVM、LVPW、IVS更高,MAD、TAD更低,差异均有统计学意义(P<0.05)。2组出生后1周新生儿EF、PAID、AID差异无统计学意义(P>0.05);研究组出生后1周新生儿较对照组MAD、TAD更低,LVPW、IVS、RAID、RVID、LAID、LVID更高,差异均有统计学意义(P<0.05)。结论 M型超声心动图技术可准确评估妊娠期糖尿病胎儿左室形态变化,可发现心功能损害,还可评估出生后新生儿心脏结构变化及心功能损害改善情况。
Objective To study the value of M-mode echocardiography in evaluating the cardiac function of fetuses and neonates with gestational diabetes mellitus(GDM). Methods Using retrospective analysis methods,50 cases of pregnant women with gestational diabetes who were admitted to the First People’s Hospital of Zigong City from June 2019 to June 2020 were selected as the research object and set as the research group;at the same time,50 cases of normal patients admitted to the hospital during the same period were selected. Pregnant women with fetal pregnancy were the research objects and were set as the control group;both groups underwent M-mode echocardiography. Compare the two groups of fetal echocardiographic index levels [left ventricular myocardial weight(LVM),left ventricular fractional shortening(LVFS),left ventricular ejection fraction(LVEF),mitral valve annulus displacement(MAD),tricuspid annulus displacement(TAD),arterial duct(AD),foramen ovale(OF),pulmonary artery inner diameter(PAID),aortic inner diameter(AID),left ventricular posterior wall(LVPW),interventricular septum(IVS),right atrium inner diameter(RAID),Right ventricular diameter(RVID),left atrial diameter(LAID),left ventricular diameter(LVID),thoracic transverse diameter(Th D),cardiac transverse diameter(TCD) ]and the echocardiographic index level of neonates 1 week after birth [Bicuspid Annulus displacement(MAD),tricuspid annulus displacement(TAD),ejection fraction(EF),pulmonary artery inner diameter(PAID),aortic inner diameter(AID),left ventricular posterior wall(LVPW),interventricular septum(IVS),Right atrium inner diameter(RAID),right ventricular inner diameter(RVID),left atrium inner diameter(LAID),left ventricular inner diameter(LVID) ]. Resluts There was no significant difference in fetal LVFS,LVEF,AD,OF,PAID,AID,RAID,RVID,LAID,LVID,THD,TCD between the two groups(P>0.05).
