女性围绝经期卵巢储备功能与血糖代谢功能相关性的研究 被引量:10

Correlation between ovarian reserve function and glucose metabolism in perimenopausal women

摘要 目的分析围绝经期女性卵巢储备功能与血糖代谢功能的相关性。方法选取2018年2月至2019年1月于重庆市第九人民医院妇科及内分泌科门诊就诊的242例围绝经期患者(围绝经期组),另选取同期240名18~39岁月经正常女性为正常对照组(NC),检测两组卵巢储备功能指标,分析卵巢储备功能与血糖代谢功能的相关性。结果与NC组比较,围绝经期组年龄、绝经率、生育率、糖尿病、妇科疾病发生率较高(P<0.05),抗苗勒管激素(AMH)、雌二醇(E_(2))、窦卵泡数(AFC)水平降低,黄体生成素(LH)、卵泡刺激素(FSH)、FPG、FIns水平升高(P<0.05)。Pearson相关性分析显示,AMH与BMI、FIns呈正相关(r=0.258、0.715,P<0.05),与FPG、AGE、AFC呈负相关(r=-0.425、-0.628、-0.648,P<0.05)。Logistic回归分析显示,FPG、FIns是卵巢储备功能不足的影响因素。结论围绝经期女性卵巢储备功能降低与血糖代谢紊乱密切相关,加强血糖监测,合理补充雌激素,纠正代谢紊乱,可降低围绝经期女性代谢性疾病发病率。 Objective To analyze the correlation between ovarian reserve function and glucose metabolism function in perimenopausal women.Methods A total of 242 perimenopausal women who visited the clinic of Department of Gynecology and Endocrinology in Chongqing Ninth People’s Hospital were selected as perimenopausal group from February 2018 to January 2019. Meanwhile,women with normal menstruation between 18 and 39 years old were selected as normal control group(NC). The index of ovarian reserve function was detected in the two groups. Then the correlation between ovarian reserve function and glucose metabolism was analyzed.Results Age,menopause rate,fertility rate,the prevalence of diabetes and gynecological diseases were higher in perimenopausal group than in NC group(P<0. 05). AMH,E_(2) and AFC levels decreased,while LH,FSH,FPG and FIns levels increased in perimenopausal group compared with NC group(P<0. 05). Pearson correlation analysis showed that AMH was positively correlated with BMI and FIns(r=0. 258,0. 715,P<0. 05),and negatively correlated with FPG,AGE and AFC(r=-0. 425,-0. 628,-0. 648,P<0. 05). Logistic regression analysis showed that FPG and FIns were the influencing factors for ovarian reserve deficiency.Conclusion The decline of ovarian reserve is closely related to glucose metabolism disorders in perimenopausal women. Strengthening blood glucose monitoring,reasonable supplementation of estrogen,and correcting metabolic disorders could reduce the incidence of metabolic diseases in perimenopausal women.
