摘要
目的:探讨清胃散合玉女煎对牙周炎大鼠牙龈组织和龈沟液炎症因子和细胞外基质金属蛋白酶的影响,分析其与牙周炎的发生发展的相关性。方法:选取健康SPF级SD大鼠60只,随机分为3组,对照组正常饲养,模型组和治疗组采用结扎法+高糖高粘饮食+涂菌法建立牙周炎模型。治疗组采用清胃散合玉女煎汤剂灌胃,0.1mL·kg-1/d,连续30d,模型组和对照组灌以等体积的无菌蒸馏水。采用PCR技术检测龈沟液中牙龈卟啉单细胞菌(Pg)、福赛类杆菌(Tf)、放线杆菌(Aa),比较各组检出率。检查各组牙周指数(GI)、附着丧失(AL)及龈沟出血指数(SBI)情况。采用酶联免疫吸附实验(ELISA)双抗体夹心法检测牙龈组织上清液和龈沟液中肿瘤坏死因子-α(TNF-α)及白细胞介素-1β(IL-1β)、IL-6的浓度。采用日立-7180全自动生化分析仪检测两组龈沟液天冬氨酸转移酶(AST)、碱性磷酸酶(ALP)、丙氨酸氨基转移酶(ALT)水平。采用酶联免疫吸附法(ELISA)测定两组基质金属蛋白酶-1(MMP-1)、基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶-9(MMP-9)的水平。结果:治疗组经治疗后,Pg、Tf、Aa检出率,GI、AL、SBI指标与对照组比较无差异,但均显著低于模型组,差异具有统计学意义(P<0.05);治疗组经治疗后,牙龈组织上清液和龈沟液中炎症因子TNF-α、IL-1β和IL-6水平明显高于对照组,但均低于模型组(P<0.05);治疗组经治疗后,AST、ALP、ALT、MMP-1、MMP-2、MMP-9水平明显高于对照组,但均显著低于模型组(P<0.05)。结论:清胃散合玉女煎中药方剂可有效消除牙周炎大鼠龈沟液中致病菌,改善牙周指标,降低炎症因子、基质金属蛋白酶水平,促进牙周组织恢复,有望成为治疗牙周病的新方法。
Objective:To investigate the effects of qingwei powder and yunyu decoction on inflammatory factors and extracellular matrix metalloproteinase in gingival tissue and gingival crevicular fluid in of rats with periodontitis,and to analyze their correlation with the occurrence and development of periodontitis.Methods:Sixty SD rats were selected and randomly divided into 3 groups,with 20 rats each.The control group was fed normally,the model group and the observation group were set up the periodontitis model by ligation method+high sugar and high viscosity diet+smear method.In addition,the observation group was treated with qingwei powder and yunyu decoction,0.1 mL·kg-1/d,while the control group and model group given the same volume DDW as solvent stimulus.The detection rates of porphyromonobacteria gingivalis(Pg),Forsytioides(Tf)and Actinobacillus gingivalis(Aa)in gingival crevicular fluid were compared by PCR.The periodontal index(GI),loss of attachment(AL)and gingival crevasse bleeding index(SBI)were examined in each group.Periodontal index(GI),attachment loss(AL)and gingival sulcus bleeding index(SBI)were examined in each group.The concentrations of tumor necrosis factor-α(TNF-α),interleukin-1(IL-1)and IL-6 in the supernatant of gingival tissue and gingival crevicular fluid were determined by enzyme-linked immunosorbent assay(ELISA).The levels of aspartic transferase(AST),alkaline phosphatase(ALP)and alanine aminotransferase(ALT)in gingival sulci fluid were measured by Hitachi-7180 automatic biochemical analyzer.The levels of matrix metalloproteinase-1(MMP-1),matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase-9(MMP-9)were determined by enzyme-linked immunosorbent assay(ELISA).Results:After treatment,the detection rates of Pg,Tf and Aa,GI,AL and SBI in the observation group were not different from those in the control group,but were significantly lower than those in the model group,with statistically significant differences(P<0.05).After treatment,the levels of inflammatory cytokines TNF-α,IL-1βand
