摘要
目的探讨奥氮平联合棕榈酸帕利哌酮治疗精神分裂症的疗效。方法将70例精神分裂症患者按照随机数字表法分为研究组与对照组各35例。研究组给予奥氮平联合棕榈酸帕利哌酮治疗,对照给予奥氮平联合利培酮治疗,观察13周。治疗前后比较两组皮质醇、泌乳素、空腹血糖、总胆固醇、三酰甘油水平,比较两组阳性与阴性症状量表评分,比较两组不良反应发生率。结果治疗后研究组皮质醇水平较治疗前显著降低(P<0.01),泌乳素水平较治疗前显著升高(P<0.01),对照组皮质醇水平较治疗前显著降低(P<0.01),泌乳素、空腹血糖、总胆固醇、三酰甘油水平较治疗前显著升高(P<0.01),研究组皮质醇、泌乳素、空腹血糖、总胆固醇、三酰甘油水平均显著低于对照组(P<0.05或0.01)。治疗后两组阳性与阴性症状量表各子量表评分及总分均较治疗前显著下降(P<0.01),研究组显著低于对照组(P<0.05或0.01)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论奥氮平联合棕榈酸帕利哌酮可有效改善精神分裂症患者的阳性与阴性症状,缓解机体的应激反应,防止性腺功能减退,降低心脑血管疾病发生风险,安全性较高。
Objective To investigate the efficacy of olanzapine combined with paliperidone palmitate in the treatment of schizophrenia.Methods A total of 70 patients with schizophrenia were divided into study group and control group according to random number table method,with 35 cases in each group.The study group was treated with olanzapine combined with paliperidone palmitate,and the control group was treated with olanzapine combined with risperidone for 13 weeks.The levels of cortisol(Cor),prolactin(PRL),fast plasma glucose(FPG),total cholesterol(TC)and triglyceride(TG)were compared between the two groups before and after treatment.The positive and negative symptom scale(PANSS)scores and the incidence of adverse reactions were compared between the two groups.Results After treatment,the Cor level in the study group was significantly lower than that before treatment(P<0.01),and the PRL level was significantly higher than that before treatment(P<0.01).The Cor level in the control group was significantly lower than that before treatment(P<0.01),and the levels of PRL,FPG,TC and TG were significantly higher than those before treatment(P<0.01).The levels of Cor,PRL,FPG,TC and TG in the study group were significantly lower than those in the control group(P<0.05 or 0.01).After treatment,the PANSS subscale scores and total scores of the two groups were significantly lower than those before treatment(P<0.01),and the study group was significantly lower than the control group(P<0.05 or 0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Olanzapine combined with paliperidone palmitate can effectively improve the positive and negative symptoms of patients with schizophrenia,alleviate the body's stress response,prevent hypogonadism,reduce the risk of cardiovascular and cerebrovascular diseases,and has high safety.
