摘要
目的研究放射性肺损伤中医证型与患者性别、年龄、美国肿瘤放射治疗协作组织(Radiation therapy oncology group,RTOG)分级的关系以及服用清肺养阴方的疗效评价。方法随机选取通用环球鞍钢集团总医院肿瘤医院2017年1月1日-2019年12月31日93例放射性肺损伤患者为研究对象,收集患者的性别、年龄、RTOG分级、影像资料及中医四诊等临床资料辨证。统计分析中医证型与患者性别、年龄、RTOG分级的关系。同时,随机抽取20例患者给予清肺养阴方口服2周,再进行生活质量评价。结果放射性肺损伤性别、RTOG分级与中医证型分布差异无统计学意义(P>0.05),不同年龄的中医证型分布差异有统计学意义(P<0.05)。放射性肺损伤以气阴两虚证和痰热壅肺证多见,其中RTOG 1~2级以气阴两虚证多见,3级以痰热壅肺证多见,4级以痰热壅肺证和气虚血瘀证多见。服用中药后,患者的生活质量有所改善(P<0.01)。结论放射性肺损伤中医证型与患者性别、RTOG分级无明显相关性;与年龄段有一定的相关性。同时,中药辅助治疗有助于提高患者的生活质量,可能为中医药治疗放射性肺损伤提供一定的参考依据,为该病中医规范化治疗打下一定基础。
Objective To study the relationship between radiation-induced lung injury syndrome type and patients’gender,age and radiation therapy oncology group(RTOG)classification,and to evaluate the efficacy of clearing lung and nourishing yin method.Methods 93 patients with radiation lung injury after radiotherapy in our hospital were selected as the research objects,and the gender,age,RTOG classification,imaging data and clinical data of four diagnosis of Traditional Chinese medicine were collected.The relationship between traditional Chinese medicine(TCM)syndromes and patients’gender,age and RTOG classification was statistically analyzed.At the same time,20 patients were randomly selected before and after radiotherapy for oral administration of clearing lung and nourishing yin method for 2 weeks,and then TCM clinical symptom score and life quality evaluation were performed.Results There were no statistically significant differences in gender,RTOG classification and TCM syndrome type distribution(P>0.05),the distribution of TCM syndromes of different ages was statistically different(P<0.05).Radioactive lung injury is mostly manifested by qi and Yin deficiency syndrome and phlegm-heat accumulation syndrome,among which RTOG level 1-2 is mostly manifested by qi and yin deficiency syndrome,level 3 by phlegm-heat accumulation syndrome,and level 4 by phlegm-heat accumulation syndrome and qi deficiency and blood stasis syndrome.TCM clinical symptom scores and quality of life were improved(P<0.05 or P<0.01).Conclusion There was no significant correlation between the TCM syndromes of radioactive lung injury and disease gender and RTOG classification.There is a certain correlation with age.At the same time,TCM assisted treatment is helpful to improve patients’quality of life.It may provide reference basis for TCM treatment of radioactive lung injury and lay a foundation for TCM standardized treatment of this disease.
