摘要
用药错误是医疗机构中最常见的危害患者安全的原因之一。管理因素、流程因素、环境因素、人员因素、设备因素和药品因素等均可导致用药错误发生。有效防范用药错误需要构建用药错误防范系统,从整个系统层面包括政策、组织、文化、信息系统、医护团队建设等进行建设。本文针对医疗机构用药错误防范系统,探讨构建用药错误防范系统的策略。
Medication errors are one of the most common causes of endangering patient safety in medical institutions.Management factors,process factors,environmental factors,personnel factors,equipment factors and drug factors can all lead to medication errors.Effective prevention of medication errors requires the establishment of a medication error prevention system,which include policy,organization,culture,information systems,and medical team building from the overall system level.This article aimed at the medication error prevention system of medical institutions,and discussed the strategy of constructing medication error prevention system.
