摘要
目的:探讨飞秒激光小切口角膜基质透镜取出术(SMILE)与飞秒激光制瓣准分子激光原位角膜磨镶术(FS-LASIK)术后干眼和角膜知觉情况。方法:本研究为前瞻性、非随机、对照研究,将2019年12月-2020年5月笔者所在医院收治的自愿接受SMILE手术的50例患者设为SMILE组,将2019年12月-2020年5月笔者所在医院收治的自愿接受FS-LASIK手术的50例患者设为FS-LASIK组。比较两组视力、屈光度、手术前后泪河高度(TMH)、平均泪膜破裂时间(BUT)和角膜知觉。结果:术前两组UDVA、主觉验光等效球镜、TMH、BUT和角膜知觉比较差异均无统计学意义(P>0.05)。术后3个月,两组UDVA、主觉验光等效球镜比较差异均无统计学意义(P>0.05)。术后3个月,两组TMH、BUT、角膜知觉均较术前降低(P<0.05)。术后3个月,SMILE组TMH、BUT、角膜知觉均优于FS-LASIK组,差异有统计学意义(P<0.05)。结论:SMILE和FS-LASIK均会造成术后干眼程度的增加和中央角膜知觉的下降,但SMILE术后干眼程度较轻,角膜知觉保留较好。
Objective:To investigate the dry eye and corneal sensation after surgery of femtosecond laser small incision lenticule extraction(SMILE)and femtosecond laser-assisted in situ keratomileusis(FS-LASIK).Method:This study was a prospective,non-randomized,controlled study.A total of 50 patients who voluntarily underwent SMILE surgery in our hospital from December 2019 to May 2020 were set as the SMILE group,and 50 patients who voluntarily underwent FS-LASIK surgery in our hospital from December 2019 to May 2020 were set as the FS-LASIK group.Visual acuity,diopter,tear meniscus height(TMH),mean tear film breakup time(BUT)and corneal sensation before and after surgery were compared between the two groups.Result:Before surgery,there were no statistically significant differences in UDVA,equivalent spherical lens of main sense optometry,TMH,BUT and corneal sensation between the two groups(P>0.05).Three months after surgery,there were no statistical significance in UDVA and equivalent spherical lens of main sense optometry between the two groups(P>0.05).Three months after surgery,TMH,BUT and corneal sensation in both groups were lower than those before surgery(P<0.05).Three months after surgery,TMH,BUT and corneal sensation in the SMILE group were better than those in the FS-LASIK group,the differences were statistically significant(P<0.05).Conclusion:Both SMILE and FS-LASIK can increase the degree of dry eye and decrease the corneal sensation after surgery,but SMILE has mild degree of dry eye after surgery and better conservation of corneal sensation.
