摘要
目的:探讨痛风性关节炎患者中性粒细胞胞外诱捕网(NETs)表达水平与骨破坏、炎症因子的相关性。方法:选择2019年1月至2020年2月在咸宁市中心医院进行治疗的63例痛风性关节炎患者作为观察组,取50名同期健康体检人员作为对照组。观察组在治疗前抽取空腹5 mL外周静脉血,对照组于体检过程中抽取空腹5 mL外周静脉血,ELISA法测定血清NETs、RANKL、TRACP5b、β-CTX、CRP、IL-6、TNF-α、PGE2、sICAM-1和sVCAM-1表达水平。结果:观察组血清NETs水平均较对照组升高(P<0.05);观察组血清CRP、IL-6、TNF-α、PGE2、sICAM-1和sVCAM-1水平均较对照组上升(P<0.05);观察组血清中骨破坏因子RANKL、TRACP5b、β-CTX较对照组升高(P<0.05);观察组血清NETs水平与CRP的相关性无统计学意义(P>0.05),与RANKL、TRACP5b、β-CTX、IL-6、TNF-α、PGE2、sICAM-1和sVCAM-1均呈正相关(P<0.05)。结论:痛风性关节炎患者急性发作期血清中NETs水平异常升高,其与炎症反应和骨质破坏有较强的相关性,是反映痛风性关节炎急性发作期的炎症水平和骨质破坏程度的良好指标。
Objective:To investigate the correlation among the expression level of neutrophil extracellular trapps(NETs),bone destruction and inflammatory factors in patients with gouty arthritis.Methods:A total of 63 patients with gouty arthritis treated in Xianning Central Hospital between January 2019 and February 2020 were included as the study group.A contemporary cohort of 50 health checkup subjects were enrolled as the control group.Fasting peripheral venous blood samples(5 mL)were collected from the study group before treatment and from the control group during the health checkup.Serum levels of NETs,RANKL,TRACP5b,β-CTX,CRP,IL-6,PGE2,sICAM-1,sVCAM-1 and TNF-αexpression were determined by ELISA.Results:The serum NETs level was higher in the study group than that in the control group(P<0.05).The serum levels of CRP,IL-6,TNF-α,PGE2,sICAM-1 and sVCAM-1 were higher in the study group compared with the control group(P<0.05).The serum levels of bone destruction related factors RANKL,TRACP5b,andβ-CTX were higher in the study group than those in the control group(P<0.05).In the study group,the serum NETs did not show significant correlation with CRP(P>0.05),but was positively correlated with levels of RANKL,TRACP5b,β-CTX,IL-6,TNF-α,PGE2,sICAM-1 and sVCAM-1(P<0.05).Conclusion:The serum level of NETs is abnormally increased in patients with gouty arthritis during an acute flare-up.In these patients,NETs is closely correlated with inflammatory response and bone destruction,and can be a useful indicator for the inflammation and bone destruction during acute flare-up of gouty arthritis.
