摘要
目的观察人类乳头瘤病毒(HPV)感染宫颈癌患者阴道菌群及宫颈炎性因子表达情况。方法选取宫颈病变患者110例作为研究对象,按照其病变程度不同将其分为宫颈癌组和癌前病变(宫颈上皮内瘤样病变)组,每组55例,比较两组HPV分型(HPV-16、HPV-18、其它型、混合型)、人类乳头瘤病毒-脱氧核糖核酸(HPA-DNA)阳性率、阴道菌群感染类型(滴虫、细菌、支原体、衣原体、霉菌、混合菌)、阴道菌群感染阳性率、炎性因子水平[干扰素γ(IFN-γ)、白细胞介素2(IL-2)、白细胞介素4(IL-4)、肿瘤坏死因子α(TNF-α)水平]。结果宫颈癌组HPV-16、HPV-18、混合型及HPV-DNA阳性患者比例显著高于癌前病变组,差异有统计学意义(P<0.05)。宫颈癌组滴虫、细菌、支原体、衣原体及菌群感染阳性患者比例显著高于癌前病变组,差异有统计学意义(P<0.05)。宫颈癌组IFN-γ、IL-2水平显著低于癌前病变组,IL-4水平显著高于癌前病变组,差异有统计学意义(P<0.05)。HPV-DNA阳性率与菌群感染阳性率、IL-4水平呈正相关,与IFN-γ、IL-2水平呈负相关(P<0.05)。结论细菌阴道、炎性因子等均有可能协同参与HPV感染宫颈癌患者疾病进展,需引起临床医师的密切关注。
Objective To study the expression of inflammatory factors and vaginal flora in cervical cancer patients infected with human papillomavirus(HPV).Methods 110 patients with cervical lesions were selected as the research object,and they were divided into cervical cancer group and precancerous lesion(Cervical intraepithelial neoplasia)group according to the degree of lesions,55 cases in each group,HPV typing(HPV-16,HPV-18,other types,mixed type),human papillomavirus-deoxyribonucleic acid(HPA-DNA)positive rate,vaginal flora infection type(Trichomonas,bacteria,mycoplasma,chlamydia,mold,mixed bacteria),vaginal flora infection positive rate,inflammatory factor level[interferonγ(IFN-γ),Interleukin 2(IL-2),Interleukin 4(IL-4),tumor necrosis factor alpha(TNF-α)levels].Results The proportion of HPV-16,HPV-18,mixed and HPV-DNA positive patients in cervical cancer group was significantly higher than that in precancerous lesion group,the difference was statistically significant(P<0.05).The proportion of patients with positive infections of trichomoniasis,bacteria,mycoplasma,chlamydia,and flora in the cervical cancer group was significantly higher than that in the precancerous lesion group,and the difference was statistically significant(P<0.05).The levels of IFN-γand IL-2 in the cervical cancer group were significantly lower than those in the precancerous lesion group,and the levels of IL-4 were significantly higher than those in the precancerous lesion group.The difference was statistically significant(P<0.05).The positive rate of HPV-DNA was positively correlated with the positive rate of bacterial infection and IL-4 level,and negatively correlated with the levels of IFN-γand IL-2(P<0.05).Conclusion Bacterial vagina and inflammatory factors may be involved in the disease progression of HPV infected cervical cancer patients,which should be paid close attention by clinicians.
