摘要
目的:探讨2.4mm锁定钢板治疗老年人桡骨远端C型骨折的临床疗效。方法:回顾性分析2015年1月~2018年12月应用2.4mm锁定内固定系统治疗28例桡骨远端骨折C型骨折患者,其中男9例,女19例,年龄60~76岁,平均66.4岁。按照AO-OTA分型C1型15侧,C2型9侧,C3型3侧。掌侧锁定钢板固定18侧,背侧锁定钢板固定10侧。结果:28例患者获得随访,随访时间为6~15个月,平均8.3个月;所有患者骨折均达到骨性愈合;术后患者腕关节活动度与健侧差异无统计学意义(P>0.05);根据改良Garland-Werley评分方法评定疗效:优16侧,良9侧,可3侧,优良率为89.3%;术后出现肌腱激惹2例,腕部慢性疼痛1例,未发现伤口感染、螺钉松动、内固定断裂等并发症。结论:采用2.4mm系统治疗老年人C型桡骨远端骨折固定牢固,可早期功能锻炼,能有效恢复桡骨远端的解剖结构,获得满意疗效。
Objective:To observe the clinical outcome of 2.4 mm locking plate system used for type C distal radius fractures in elderly patients.Methods:From Jan 2015 to October 2018,28 patients with type C distal radius fractures were managed with 2.4 mm locking plate system,who were 9 men and 19 women,aged from 60 to 76 years(average,66.4 years).According to AO-OTA classification there were 15 fractures of type C1, 9 type C2 and 3 type C3.Volar plating was conducted in 18 cases and dorsal plating in 10 cases.Results:28 patients were followed up from 6 months to 15 months(average,8.3 months);union was obtained in all patients;the average active wrist motion were similar to the contralateral side(P>0.05);modified Gartland-Werley scores were excellent in 16 patients,good in 9 patients and fair in 3 patients,with an excellent to good rate of 89.3%;there were two patients with tendon irritation and a complex regional pain;no incision infection,screw loosening,fracture fixation failure complications were observed.Conclusion:Treatment of type C distal radius fractures in elderly patients with 2.4 mm locking plate can achieve rigid fixation and rapid rehabilitation,leading to satisfactory clinic outcomes and minimal soft tissue complications.
