急性缺血性脑卒中的microRNA-16表达水平及其对预后的影响 被引量:2

摘要 目的探讨microRNA-16(miR-16)在急性缺血性脑卒中(acute ischemic stroke,AIS)中表达水平及其对预后的影响。方法选取辽阳市中心医院神经内科2016年1月-2020年1月收治的279例AIS患者(AIS组)作为研究对象,治疗结束后3个月,采用改良Rankin量表(modified rankin scale,MRS)评价患者预后,根据预后情况将患者分为预后良好组(196例)和预后不良组(83例)。另选取114例在本院例行体检的健康者作为健康组。采用实时荧光定量PCR(quantitative real-time PCR,qRT-PCR)检测血清中miR-16含量。比较AIS组和健康组外周血miR-16表达水平;比较预后不良组和预后良好组患者一般临床资料及生化指标;多因素Logistic回归分析预后不良的危险因素;受试者工作特征曲线(receiver operating characteristic curve,ROC)分析外周血miR-16表达水平对AIS预后不良的预测价值。结果AIS组外周血miR-16、miR-135b表达水平显著高于健康组(P<0.05);预后不良组患者年龄,糖尿病史率,外周血中miR-16、miR-135b表达水平,MRS评分,美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分均显著高于预后良好组(均P<0.05);年龄、糖尿病史、miR-16、miR-135b、MRS、NIHSS评分为AIS患者预后不良的影响因素(均P<0.05);miR-16、miR-135b、miR-16联合miR-135b预测AIS预后不良的曲线下面积(area under the curve,AUC)分别为0.739(95%CI=0.676~0.801)、0.704(95%CI=0.638~0.770),0.802(95%CI=0.745~0.859);miR-16联合miR-135b预测AIS预后不良的AUC大于miR-16、miR-135b的AUC(P<0.05)。结论miR-16在AIS中表达水平升高,升高预示患者预后不良,可作为预测AIS预后不良的标志物,且miR-16联合miR 135b对AIS患者预后不良的测价值高于单独miR-16、miR-135b。 Objective To investigate the expression level of microRNA-16(miR-16)in acute ischemic stroke(AIS)and its influence on prognosis.Methods A total of 279 patients with AIS(AIS group)admitted to the Department of Neurology of Liaoyang Central Hospital from January 2016 to January 2020 were selected as the research objects.Three months after the treatment,the modified Rankin Scale(MRS)was used to evaluate the prognosis of patients.Ac‐cording to the prognosis,the patients were divided into a good prognosis group(196 cases)and poor prognosis group(83 cases).In addition,114 healthy patients who received routine physical examination in our hospital were selected as the healthy group.Quantitative real-time PCR(qPCR)was used to detect the level of miR-16 in serum.The expression lev‐el of peripheral blood miR16 in the AIS group and the healthy group were compared.General clinical data and bio‐chemical indicators of patients in the poor prognosis group and the good prognosis group were compared.Multivariate Logistic regression was used to analyze the risk factors for poor prognosis.Receiver operating characteristic curve(ROC)was used to analyzethe predictive value of peripheral blood miR-16 expression level for poor prognosis of AIS.Results The expression levels of peripheral blood miR-16 and miR-135b in the AIS group were significantly higher than those in the healthy group(P<0.05).The age,history rate of diabetes,serum expression levels of miR-16 and miR-135b,MRS score,andNational Institute of Health stroke scale(NIHSS)score of the poor prognosis groupwere all significantly higher than those of the good prognosis group(all P<0.05).Age,diabetes history,miR-16 level,miR-135b level,NI‐HSS score,and MRS score were all the factors for poor prognosis in AIS patients(all P<0.05).The AUC values of miR 16,miR-135b,and miR-16 combined with miR-135b for predicting the poor prognosis of AIC was 0.739(95%CI=0.676~0.801),0.704(95%CI=0.638~0.770),and 0.802(95%CI=0.745~0.859)respectively.The AUC of miR-16 com‐bined with miR135b f
