摘要
目的:观察愈胆汤联合羟甲香豆素片治疗胆囊切除术后综合征(PCS)临床疗效。方法:选取96例PCS患者,按随机数字表法分为对照组及观察组各48例。对照组给予羟甲香豆素片治疗,观察组给予羟甲香豆素片联合愈胆汤治疗。2组均治疗4周。比较2组临床疗效与不良反应发生率,观察2组治疗前后中医证候积分、胃肠激素[血清胆囊收缩素(CCK)、胃动素(MTL)]、生化指标[总胆红素(TBil)、直接胆红素(DBil)]的变化。结果:治疗后,观察组总有效率为91.67%,对照组总有效率为64.58%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组术后疼痛、恶心呕吐、食欲障碍、腹泻积分均低于治疗前(P<0.05),观察组上述4项积分均低于对照组(P<0.05)。治疗后,2组CCK、MTL水平均高于治疗前(P<0.05),观察组上述2项水平均高于对照组(P<0.05)。治疗后,2组TBil、DBil水平均较治疗前降低(P<0.05),观察组上述2项水平均低于对照组(P<0.05)。观察组不良反应发生率为6.25%,对照组不良反应发生率为12.50%,2组比较,差异无统计学意义(P>0.05)。结论:愈胆汤联合羟甲香豆素片治疗PCS能够促进胆汁排泄,减少胆汁瘀积,改善胃肠功能,缓解临床症状。
Objective: To observe the clinical effect of Yudan tang combined with hymecromone tablets for postcholecystectomy syndrome(PCS). Methods: A total of 96 cases of PCS patients were selected and divided into the control group and the observation group according to the random number table method,48 cases in each group. The control group was treated with hymecromone tablets,and the observation group was treated with hymecromone tablets combined with Yudan tang. Both groups were treated for four weeks. The clinical effect and the incidences of adverse reactions in the two groups were compared. Before and after treatment,the changes in symptom scores,gastrointestinal hormones and biochemical indexes in the two groups were observed. Results:After treatment,the total effective rate was 91.67% in the observation group and 64.58% in the control group,the difference being significant(P<0.05). After treatment,the scores of pain,nausea and vomiting,dysorexia and diarrhea after operation in the two groups were lower than those in the control group(P<0.05), and the four scores in the observation group were lower than those in the control group(P<0.05). After treatment,the levels of CCK and MTL in the two groups were higher than those before treatment(P<0.05),and the two levels in the observation group were higher than those in the control group(P<0.05). After treatment,the levels of TBil and DBil in the two groups were lower than those in the control group(P<0.05),and the two levels in the observation group were lower than those in the control group(P<0.05). The incidence of adverse reactions was 6.25% in the observation group and12.50% in the control group,there being no significance in the difference(P>0.05). Conclusion:Yudan tang combined with hymecromone tablets for PCS can improve clinical symptoms,promote biliary excretion,reduce bile deposition and improve gastrointestinal function.
