摘要
目的分析以医护一体化管理模式对糖尿病患者在安全管理中的应用效果。方法将2019年1~6月在陕西省人民医院内分泌科住院的1788例糖尿病患者设为对照组,7~12月住院的1870例糖尿病患者设为干预组。对照组实施传统常规诊疗护理,干预组以医护一体化管理模式进行干预。比较两组患者住院期间护理不良事件发生率、低血糖发生率及胰岛素安全注射规范率。结果干预组患者住院期间不良事件发生率和低血糖发生率分别为0.27%、1.87%,明显低于对照组的0.73%、3.13%,差异均有统计学意义(P<0.05);虽然干预组患者的皮肤消毒规范率为86.38%,略高于对照组的81.66%,差异无统计学意义(P>0.05),但干预组患者在正确评估注射部位、规范轮换注射部位、针头一次性使用、注射后留置时间规范、医废规范处置等方面的规范率分别为79.57%、56.37%、72.38%、88.65%、42.50%,明显高于对照组的64.81%、47.95%、60.71%、78.82%、29.36%,差异均有统计学意义(P<0.01)。结论对住院糖尿病患者实施医护一体化管理模式干预能够显著降低医疗护理风险,提高患者安全管理,值得临床推广应用。
Objective To analyze the application effect of integrated medical and nursing management model for diabetic patients in safety risk management.Methods A total of 1788 diabetic patients hospitalized in Department of Endocrinology of Shaanxi Provincial People's Hospital from January to June 2019 were set as the control group,and 1870 diabetic patients hospitalized from July to December were set as the intervention group.The control group received tradi-tional routine diagnosis and treatment,and the intervention group applied integrated medical and nursing management model for intervention.The incidence of nursing adverse events,the incidence of hypoglycemia,and the standardized rate of safe insulin injection during hospitalization between the two groups were compared.Results The incidence of adverse events and hypoglycemia during hospitalization in the intervention group were 0.27%and 1.87%,respectively,which were significantly lower than corresponding 0.73%and 3.13%of the control group(P<0.05);the standard rate of skin disinfection in the intervention group was 86.38%,which was slightly higher than 81.66%in the control group(P>0.05);the standardization rates in the correct assessment of injection sites,standardized rotation of injection sites,dispos-able use of needles,standardized retention time after injection,and standardized disposal of medical waste were 79.57%,56.37%,72.38%,88.65%,42.50%in the intervention group,respectively,which were significantly higher than corre-sponding 64.81%,47.95%,60.71%,78.82%,29.36%of the control group(all P<0.01).Conclusion The implementa-tion of integrated medical and nursing management model for hospitalized diabetic patients can significantly reduce med-ical care risks and improve patient safety management,which is worthy of clinical application.
