摘要
目的探讨快速康复理念指导下右美托咪定对老年结直肠癌患者术后的镇痛效果。方法选择我院从2018年3月至2020年9月收治的230例行腹腔镜手术的老年结直肠癌患者,按照随机数字表分为对照组和观察组,各115例。对照组给予生理盐水辅助全麻,观察组给予右美托咪定辅助全麻。观察两组患者术后不同时间的镇痛效果、不同时间血流动力学指标、术后下床活动时间、胃肠功能恢复时间、术后住院天数及术后不良反应的发生情况。结果观察组术后4 h、8 h、12 h、24 h、48 h疼痛评分均低于对照组(均P<0.05);术后镇痛药物补救率观察组为13.9%,对照组为24.3%,对照组高于观察组(χ2=4.047,P<0.05);Ramesay评分均高于对照组(P<0.05);术后心率、血压波动幅度均低于对照组(P<0.05);观察组总不良反应发生率为11.3%、对照组为24.3%,对照组高于观察组(χ2=6.678,P<0.05)。结论快速康复理念指导下右美托咪定可提高老年结直肠癌患者术后的镇痛效果,降低不良反应发生率,且血流动力学较稳定。
Objective To explore the postoperative analgesic effect of Dexmedetomidine on elderly patients with colorectal cancer under the guidance of the concept of rapid recovery after surgery.Methods A total of 230 elderly patients with colorectal cancer who underwent laparoscopic surgery in our hospital from March 2018 to September 2020 were randomly divided into an observation group(receiving Dexmedetomidine auxiliary general anesthesia,n=115)with aged(66.6±4.6)years,male 59,and control group(receiving normal saline auxiliary general anesthesia,n=115),with aged.(67.0±4.6)years,male 61.The analgesic effect,hemodyna mic index,postoperativeout of bed activity time,gastrointestinal fuction recovery time postoperative hospital stay and adverse reactions were observed.Results The pain scores at 4,8,12,24 and 48 h after operation were lower in the observation group than in control group(all P<0.05).The recovery rate of postoperative analgesic drugs was 13.9%in the observation group and 24.3%in the control group(χ^(2)=4.047,P<0.05).Ramesay scores were higher in the observation group than in the control group(P<0.05).Fluctuations of postoperative heart rate and blood pressure were lower in the observation group than in the control group(P<0.05).The total incidence of adverse reactions was lower in the observation group(11.3%)than in the control group(24.3%)(χ^(2)=6.678,P<0.05).Conclusions Under the guidance of the concept of rapid recovery after surgery,Dexmedetomidine can improve the postoperative analgesic effect in elderly patients with colorectal cancer,reduce the incidence of adverse reactions,and have stable hemodynamics.
