摘要
《金匮要略》温经汤常用以治疗冲任虚寒、瘀血阻滞所致月经失调或不孕症等妇科疾病。因《金匮要略》原文所述多种症状与“寒证”无关,故“寒”可能并非温经汤证的主要病机。首先,“下利数十日”的病因不拘于寒证,还包括实热积滞和湿阻气滞;其次,“暮即发热”“手掌烦热”多责之阴虚火旺而非寒证;最后,“带下”泛指妇科疾病,非狭义寒湿带下。因冲任脉虚导致气机逆上,可引起呕逆、腹满、下利;冲任脉瘀可造成少腹里急、手掌烦热、口唇干燥,故温经汤的治疗重点在于调节冲任二脉之虚与瘀,与寒热关系不明显。
Wenjing Decoction from Jin Gui Yao Lue is commonly used to treat gynecological diseases such as menstrual disorders or infertility caused by deficient cold of thoroughfare and conception channels and static blood obstruction and stagnation.However,the various symptoms described in the original text of Jin Gui Yao Lue have nothing to do with"Cold Syndrome",and whether the main pathogenesis of Wenjingtang syndrome is"cold"remains to be discussed.First of all,the etiology of"dysentery and diarrhea for dozens of days"is not limited to the cold syndrome,but also includes excessive Heat accumulation and retention of Dampness due to stagnation of Qi.Secondly,"Fever at dusk"and"restless fever of palms"are more induced by hyperactivity of Fire due to Yin deficiency rather than Cold Syndromes.Finally,"morbid vaginal discharge"refers to gynecological diseases in general,not only the cold Wetness induced leukorrhea in the narrow sense.Deficiency of the Chong and Ren channels leads to Qi inversion,which can cause vomiting,the fullness of the abdomen,dysentery,and diarrhea.Stasis of Chong and Ren channels can cause abdomen pain of the lateral lower abdomen,restless fever of palms,and dry lips.Therefore,the treatment of Wenjing Decoction focuses on regulating the deficiency and blood stasis of the Chong and Ren channels and has no obvious relationship with cold and heat syndromes.
