互动式头针治疗中风后痉挛性瘫痪的系统评价与Meta分析 被引量:8

Systemic review and Meta-analysis of effectiveness of interactive scalp acupuncture for spastic paralysis after stroke

摘要 目的系统评价互动式头针治疗中风后痉挛性瘫痪的临床疗效。方法计算机检索建库至2020年12月31日中国期刊全文数据库(CNKI)、万方知识服务平台(WF)、维普数据库(VIP)、中国生物医学文献数据库(CBM)、The Cochrane library、Pubmed、Embase数据库中互动式头针治疗中风后痉挛性瘫痪的随机对照试验(RCTs)。在2名研究者独立完成文献筛选和方法学质量评估后,收集纳入研究的Fugl-Meyer评定量表(FMA)、改良Barthel指数(MBI)评分量表,采用Reviw Manager5.4进行Meta分析。结果共纳入17个研究,共1519例受试者,Meta分析结果显示:与康复训练相比,互动式头针可显著改善中风后痉挛性瘫痪患者的FMA评分,[MD=11.79,95%CI(9.53,14.06),P<0.00001];与康复训练相比,互动式头针可显著改善中风后痉挛性瘫痪患者的MBI评分,[MD=13.25,95%CI(8.14,18.37),P<0.00001]。与头针+康复相比,互动式头针可显著改善中风后痉挛性瘫痪患者的FMA评分,[MD=8.61,95%CI(6.11,11.11),P<0.00001];与头针+康复相比,互动式头针可显著改善中风后痉挛性瘫痪患者的MBI评分[MD=9.97,95%CI(5.60,14.34),P<0.00001]。结论根据目前证据表明,互动式头针与康复训练相比可以显著改善中风后痉挛性瘫痪患者的肢体活动功能和日常生活能力;互动式头针与头针+康复相比,互动式头针可以显著改善中风后痉挛性瘫痪患者的运动功能和日常生活能力。由于纳入研究的样本量有限,文献质量一般,故研究结论还有待高质量临床研究进行验证。 Objective This study aims to systematically review the clinical efficacy of interactive scalp acupuncture in the treatment of spastic paralysis after stroke.Methods The databases of CNKI,WF,VIP,CBM,the Cochrane Library,PubMed and EMBASE were searched for randomized controlled trials(RCTs)of interactive scalp acupuncture in the treatment of spastic paralysis after stroke until April 1,2020.After two researchers independently completed literature screening and methodological quality evaluation,Fugl-Meyer Rating Scale(FMA)and modified Barthel Index(MBI)were collected for Meta-analysis.Results A total of 17 studies were included and 1519 subjects involved.The results of Meta-analysis showed that compared with rehabilitation training,interactive scalp acupuncture could significantly improve FMA score of spastic paralysis patients after stroke[MD=11.79,95%CI(9.53,14.06)],P<0.00001,and compared with rehabilitation training,interactive scalp acupuncture could significantly improve MBI score of spastic paralysis patients after stroke[MD=13.25,(95%CI 8.14,18.37)],P<0.00001;compared with scalp acupuncture plus rehabilitation,interactive scalp acupuncture can significantly improve FMA score of spastic paralysis patients after stroke[MD=8.61,(95%CI 6.11,11.11)],P<0.00001,and compared with scalp acupuncture plus rehabilitation,interactive scalp acupuncture can significantly improve MBI score of spastic paralysis patients after stroke[MD=9.97,(95%CI 5.60,14.34)],P<0.00001.Conclusion According to the current evidence,compared with rehabilitation training,interactive scalp acupuncture can significantly improve the limb activity function and activities of daily living of patients with spastic paralysis after stroke;compared with scalp acupuncture plus rehabilitation,interactive scalp acupuncture can significantly improve the motor function and activities of daily living of patients with spastic paralysis after stroke.Due to the limited sample size and the general quality of the literature,the research conclusions need to be verifi
