摘要
目的探讨腰舒止痹方治疗肾虚寒湿型腰椎间盘突出症的临床疗效及对炎性细胞因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]、致痛物质[神经肽(NPY)、P物质(SP)]表达水平的影响。方法将2019年8月—2020年10月在黑龙江中医药大学附属第一医院治疗的110例肾虚寒湿型腰椎间盘突出症患者随机分成2组,对照组55例给予推拿+牵引治疗,观察组55例在此基础上联合腰舒止痹方治疗,2组均治疗4周。观察2组治疗前后日本骨科协会评估治疗(JOA)评分、视觉模拟疼痛(VAS)评分、Oswestry功能障碍指数(ODI)及血清TNF-α、IL-6、NPY、SP水平,记录2组总有效率及治疗过程中不良反应发生情况。结果治疗后2组VAS评分、ODI评分及血清TNF-α、IL-6、NPY、SP水平均明显降低(P均<0.05),且观察组治疗后均明显低于对照组(P均<0.05);治疗后2组JOA评分均明显升高(P均<0.05),且观察组高于对照组(P<0.05);治疗后观察组总有效率为85.5%(38/55),明显高于对照组的69.1%(47/55)(P<0.05);2组均未出现与治疗相关的不良反应。结论腰舒止痹方能可能通过降低血清中炎症因子与致痛物质水平,显著减轻肾虚寒湿型腰椎间盘突出症患者疼痛,改善腰部功能。
Objective It is to investigate the clinical efficacy of Yaoshu Zhibi Decoction in the treatment of lumbar disc herniation with syndrome of kidney-deficiency and cold-dampness,and its influence on the expression levels of inflammatory cytokines[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],pain-causing substances[neuropeptide(NPY),substance P(SP)].Methods A total of 110 patients with lumbar disc herniation with syndrome of kidney-deficiency and cold-dampness treated in the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from August 2019 to November 2020 were randomly divided into 2 groups.55 cases in the control group was treated with massage+traction,and 55 cases in the observation group were treated with Yaoshu Zhibi Decoction based on the control group,both groups were treated for 4 weeks.The scores of the Japanese Orthopaedic Association Assessment(JOA),visual analog pain(VAS),Oswestry dysfunction index(ODI)and the levels of serum TNF-α,IL-6,NPY,SP were observed before and after treatment in the 2 groups,and the total effective rate and the occurrence of adverse reactions during treatment were recorded.Results The VAS score,ODI score and the levels of serum TNF-α,IL-6,NPY,SP in the two groups were significantly reduced after treatment(all P<0.05),and the observation group was significantly lower than the control group after treatment(P<0.05);after treatment,the JOA scores of the two groups were significantly increased(P<0.05),and the observation group was higher than the control group(P<0.05);the total effective rate of the observation group after treatment was 85.5%(38/55),which was significantly higher than the control group’s 69.1%(47/55)(P<0.05);there was no treatment-related adverse reactions in both groups.Conclusion Yaoshu Zhibi Decoction can significantly reduce the pain of patients with lumbar disc herniation with syndrome of kidney-deficiency and cold-dampness and improve waist function by reducing the levels of inflammatory factors and pain-causing su
