摘要
目的分析生殖道沙眼衣原体感染、基因型分布及其对生殖道菌群和妊娠结局的影响。方法选取承德市中心医院妊娠期女性1220例,依据是否感染沙眼衣原体将其分为感染组、未感染组,比较其基线资料、生殖道菌群和妊娠结局,巢式聚合酶链反应(PCR)检测沙眼衣原体外膜蛋白A,分析其基因型分布,比较不同基因型分布的沙眼衣原体感染患者生殖道菌群和妊娠结局。结果61例妊娠期女性发生沙眼衣原体感染(5.00%),感染组白细胞计数、血小板计数高于未感染组(P<0.05);感染组阴道菌群密集度、菌群多样性、炎症反应与未感染组比较差异有统计学意义(P<0.05);感染组胎膜早破、产褥病、早产、低出生体质量儿、新生儿肺炎、新生儿感染发生率高于未感染组(P<0.05);61例沙眼衣原体感染孕妇中单基因型占60.65%(D型、E型、F型分别占13.11%、22.95%、14.75%),双重基因型感染、多重基因型感染分别占29.51%、9.84%;不同基因型分布的沙眼衣原体感染孕妇生殖道菌群比较,无统计学差异;单基因型沙眼衣原体感染孕妇胎膜早破、早产、新生儿肺炎发生率低于双重基因型感染、多重基因型感染孕妇(P<0.05)。结论生殖道沙眼衣原体感染对孕妇生殖道菌群及妊娠结局有一定影响,且沙眼衣原体感染基因型分布有一定特征,其中双重基因型、多重基因型感染患者的妊娠结局受影响更大。
OBJECTIVE To analyze the influence of genital tract Chlamydia trachomatis and genotypes on genital tract flora and pregnancy outcomes.METHODS A total of 1220 pregnant women who were treated in Chengde Central Hospital were enrolled in the study and divided into the infection group and the non-infection group.The baseline data,genital tract flora and pregnancy outcomes were compared,the outer membrane protein of Chlamydia trachomatis was detected by nested polymerase chain reaction(PCR),the genotypes were compared,and the genital tract flora and pregnancy outcomes were compared among the C.trachomatis infection patients with different genotypes.RESULTS Totally 61 pregnant women had C.trachomatis infection,accounting for 5.00%.The white blood cell counts and platelet counts of the infection group were significantly higher than those of the non-infection group(P<0.05).There were significant differences in the density of vaginal bacterial flora,diversity of flora and inflammatory reactions between the infection group and the non-infection group(P<0.05).The incidence rates of premature rupture of membranes,puerperalism,premature delivery,low birth weight,neonatal pneumonia and neonatal infection were significantly higher in the infection group than in the non-infection group(P<0.05).Among the 61 pregnant women with C.trachomatis infection,the pregnant women with single genotype accounted for 60.65%,(genotype D 13.11%,genotype E 22.95%,genotype F 14.75%);the pregnant women who had dual-genotype infection accounted for 29.51%,and the pregnant women with multiple-genotype infection accounted for 9.84%.There was no significant difference in the genital tract flora among the pregnant women with different genotypes of C.trachomatis infection.The incidence rates of premature rupture of membranes,premature delivery and neonatal pneumonia were lower among the pregnant women with single genotype C.trachomatis infection than among the pregnant women with dual-genotype and multiple-genotype C.trachomatis infection(P<0.05).CONCLUS
