首发精神分裂症患者非稳态负荷及其与认知损害的关系 被引量:5

Allostatic load and its relationship with cognitive deficits in patients with first-episode schizophrenia

摘要 目的:探讨精神分裂症患者的非稳态负荷(AL)及其与认知损害的关系。方法:纳入69例符合DSM-IV诊断标准的首发精神分裂症患者及111例正常对照。收集心血管系统、代谢系统、免疫系统、神经内分泌系统的13个生物学指标以计算AL;使用中文版精神分裂症认知功能成套测验(MCCB)评估认知功能。结果:精神分裂症组AL高于正常对照组[(5.9±1.7)vs.(3.5±2.2)],MCCB测验中的信息处理速度、注意/警觉性、言语记忆、视觉记忆、推理/问题解决能力维度得分均低于对照组(均P<0.0071,Bonferroni校正);精神分裂症组AL与MCCB测验中的推理/问题解决能力得分负相关(r=-0.37,P<0.01),正常对照组相关性无统计学意义(P>0.05)。多重逐步线性回归结果提示腰臀比与MCCB测验中的推理/问题解决能力得分负向关联(β=-0.19,P<0.01)。结论:首发精神分裂症患者非稳态负荷高于正常对照,并可能与推理/问题解决能力损害有关。 Objective:To explore allostatic load(AL) and its relationship with cognitive deficits in patients with schizophrenia.Methods:Sixty-nine patients with first-episode schizophrenia and 111 normal controls were recruited.The diagnosis was made according to the criteria of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition(DSM-IV).Thirteen biological indicators involved in cardiovascular, metabolic system, immune and neuroendocrine system were collected to calculate AL.The Chinese version of the Measurement and Treatment Research to Improve Cognition in Schizophrenia(MATRICS) Consensus Cognitive Battery(MCCB) was used to assess cognitive function.Results:Patients with schizophrenia had higher AL when compared to normal controls [(5.9±1.7) vs.(3.5±2.2)],and had lower processing speed, attention and vigilance, verbal learning, visual learning, reasoning and problem-solving scores than controls measured by MCCB(Ps<0.0071,Bonferroni correction).AL was negatively correlated with reasoning and problem-solving ability in patients with schizophrenia(r=-0.37,P<0.01),while the correlation was not significant in normal controls(P>0.05).Multiple stepwise linear regression model showed waist-hip ratio was negatively correlated with reasoning and problem-solving ability which acquired by MCCB(β=-0.19,P<0.01).Conclusion:It suggests that allostatic load is significantly higher in patients with first-episode schizophrenia than in controls, and the higher level of AL may be related to the impairment of reasoning and problem-solving ability in patients with schizophrenia.
