老年患者下呼吸道感染铜绿假单胞菌检测与耐药性分析

摘要 目的分析老年患者下呼吸道感染铜绿假单胞菌检测与耐药性。方法现将2019年3月—2020年3月期间该院收治的110例老年下呼吸道感患者作为研究对象,对其痰标本进行培养和检测并分离出铜绿假单胞菌,将铜绿假单胞菌使用全自动细菌鉴定及药敏分析系统进行鉴定,并对其进行体外药敏试验,观察其药敏情况。结果从110例老年患者下呼吸道感患者中共计分离出110株铜绿假单胞菌,对抗生素耐药率从高到低依次为头孢噻肟(71.82%)、磺胺甲噁唑/甲氧苄啶(70.91%)、庆大霉素(55.45%)、哌拉西林(52.73%)、环丙沙星(39.90%)、头孢哌酮(30.91%)、头孢吡肟(24.55%)、阿米卡星(17.27%)、氨曲南(10.91%)、哌拉西林/舒巴坦(9.09%)、头孢哌酮/舒巴坦(7.27%)、亚胺培南(0.00%)和美罗培南(0.00%),其中对于亚胺培南和美罗培南的耐药性最低,对头孢耐噻肟的耐药性最高。结论铜绿假单胞菌对于亚胺培南和美罗培南无耐药性,具有较好的敏感性,在老年患者下呼吸道铜绿假单胞菌感染的治疗中可将其作为首选药物,在联合用药时可根据药敏结果进行合理用药。 Objective To analyze the detection and drug resistance of Pseudomonas aeruginosa in lower respiratory tract infections in elderly patients.Methods 110 elderly patients with lower respiratory tract infection admitted to the hospital from March 2019 to March 2020 are now used as the research objects.Sputum samples are cultured and tested,and Pseudomonas aeruginosa is isolated.The monospores were identified using a fully automated bacterial identification and drug sensitivity analysis system,and they are subjected to in vitro drug susceptibility tests to observe their drug susceptibility.Results A total of 110 strains of Pseudomonas aeruginosa were isolated from 110 elderly patients with lower respiratory tract infection.The antibiotic resistance rate from high to low was cefotaxime(71.82%)and sulfamethoxazole/trimethoprim.Pyridine(70.91%),gentamicin(55.45%),piperacillin(52.73%),ciprofloxacin(39.90%),cefoperazone(30.91%),cephalosporin Pyroxime(24.55%),amikacin(17.27%),aztreonam(10.91%),piperacillin/sulbactam(9.09%),cefoperazone/sulbactam(7.27%),imipenem(0.00%)and meropenem(0.00%),among which the resistance to imipenem and meropenem is the lowest,and the resistance to cefotaxime was the highest.Conclusion Pseudomonas aeruginosa is not resistant to imipenem and meropenem and has good sensitivity.It can be used as the first choice drug in the treatment of lower respiratory tract Pseudomonas aeruginosa infections in elderly patients,and can be used in combination.The drug can be used rationally according to the results of drug sensitivity.
