摘要
目的动态观察妊娠合并慢性高血压患者应用小剂量阿司匹林后对凝血指标血小板聚集率(PagR)的影响。方法 106例患者为2015年1月—2018年12月期间就诊于唐山市协和医院的妊娠合并慢性高血压孕妇,根据治疗方法不同分为二组。52例常规治疗为对照组,54例常规治疗同时服用阿司匹林为观察组。比较二组患者用药前和停药前1d血四项[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)]、血粘度和PagR;观察组分别在应用阿司匹林前、服药4周、停药前1 d和停药后3d、7 d各测定1次PagR。结果治疗前二组患者PT、APTT、TT、FIB、血粘度、PagR及基线资料无统计学意义(P>0.05);治疗后观察组与对照组比较,PT、APTT、TT明显延长,而FIB、血粘度和PagR明显降低,差异均有统计学意义(均P<0.01);观察组随着阿司匹林应用时间的延长其PagR逐渐降低,差异有统计学意义(P<0.01),停药后PagR逐渐增加。结论阿司匹林可有效改善妊娠合并慢性高血压患者的凝血功能,降低血小板聚集率。
Objective To dynamically observe the effect of Low-dose aspirin on the coagulation parameters platelet aggregation in pregnancy with chronic hypertension.Methods A total of one hundred and six pregnancies with chronic hypertension admitted to Tangshan Union Medical College Hospital from January 2015 to December 2018.Divided into two groups according to different treatments.52 Patients in the control group were given conventional treatment, while 54 patients in observation group were given the conventional treatment and aspirin.We assessed the change of four items of coagulation, including prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT) and fibrinogen(FIB),as well as blood viscosity and platelet aggregation rate(PagR) before treatment and the day before withdrawal.During the treatment, PagR were measured before the treatment, at four week, the day before withdrawal, three day and seven day after withdrawal in the observation group.Results Comparisons of PT,APTT,TT,FIB and PagR between the two groups before the treatment exhibited no statistically significant differences(P>0.05).Compared with the control group, observed group demonstrated a statistically significant longer in the PT,APTT and TT.While FIB,blood viscosity and PagR were statistically significant reduction(P<0.01) after aspirin treatment.Furthermore, the PagR was decreased with increased durations of aspirin treatment in observation group, the difference was statistically significant(P<0.01) and progressively increases after withdrawal.Conclusion Aspirin treatment can improve the coagulation function of pregnancy with chronic hypertension and decrease the platelet aggregation.
