摘要
目的探讨牙列缺失/缺损老年患者种植固定修复的可行性和诊疗流程。方法选择3例因牙周病导致牙列缺失/缺损患者,根据检查评估医患交流,其中2例保留了经过牙周系统治疗的少数患牙,1例拔除全部余留牙;分别植入6~8枚植体,3~6个月重建咬合关系,行种植固定义齿修复。种植外科及修复过程中注意颌位关系、垂直距离。结果修复即刻及修复后6个月复诊,患者残留自然牙牙周状态稳定,种植体无明显边缘骨吸收,修复体无崩瓷等明显并发症。患者表示对功能美观等基本满意。但是保留部分对颌自然牙的患者,虽然调合,但仍有个别种植修复体临床冠相对较短,合曲线略不调。结论对于牙周病导致牙列缺失/缺损的患者,如果全身、口腔及经济条件允许,种植固定修复可以有效恢复患者的咀嚼功能,改善患者容貌,达到美观需求。临床效果可靠,是一种值得推广的修复方式。
Objective To explore the feasibility and process of implant-supported fixed restoration in elderly patients with dentition loss/defect.Methods Three patients with dentition loss/defect due to periodontal disease were selected,and the doctorpatient communication was evaluated according to the examination.Two of the patients with residual teeth received periodontal systemic treatment,and one had all the residual teeth extracted before 6 and 8 implants were completed respectively.Occlusion was reconstructed 3 to 6 months after implants-supported fixed restoration.Special attention was paid to the maxillo-mandibular relationship and vertical distance during implant surgery and restoration.Results After immediate restoration and 6 months after restoration,the patients’residual natural teeth resumed healthy and stable periodontal status,the implants had no obvious marginal bone resorption,and the restoration had no obvious complications such as porcelain collapse.The patients were satisfied with the function and aesthetics after implant-supported fixed restoration.However,the two patients with residual teeth still had occlusion problems despite occlusal adjustment after restoration.Conclusion For patients with dentition loss/defect caused by periodontal disease,implant-supported fixed restoration is preferable if they are in good health,orally healthy and financially well-off.This approach can effectively restore the patient’s tooth function and aesthetics.The clinical effect of restoration is reliable.
