摘要
目的对比腹腔镜与腹部小切口阑尾切除术治疗急性阑尾炎的临床治疗效果。方法选取该院2018年1月—2020年1月间收治的114例急性阑尾炎患者作为研究对象,根据不同治疗方案分为A组以及B组,每组57例。A组采取腹腔镜术治疗,B组采取腹部小切口阑尾切除术治疗,比较A、B两组患者术中出血量、术后首次排气时间、住院时间、术后VAS评分以及术后并发症发生率。结果A组术中出血量低于B组,差异有统计学意义(t=13.308,P=0.001);A组术后首次排气时间短于B组,差异有统计学意义(t=12.646,P=0.001);A组住院时间短于B组,差异有统计学意义(t=8.671,P=0.001);A组VAS评分低于B组,差异有统计学意义(t=11.438,P=0.001)。A组术后并发症发生率(5.26%)明显低于B组(17.54%),差异有统计学意义(χ^(2)=4.254,P=0.039)。结论腹腔镜治疗急性阑尾炎较腹部小切口阑尾切除术具有一定优势,术中出血量更少,术后恢复较快,术后并发症少,具有更好的适用性,值得推广。
Objective To compare the clinical effects of laparoscopic and small abdominal incision appendectomy in the treatment of acute appendicitis.Methods A total of 114 patients with acute appendicitis who were admitted to the hospital from January 2018 to January 2020 were selected as the research objects.According to different treatment plans,they were divided into group A and group B,with 57 cases in each group.Group A was treated with laparoscopic surgery,and group B was treated with a small abdominal incision appendectomy.Intraoperative blood loss,first postoperative exhaust time,hospital stay,postoperative VAS score,and postoperative complications incidence rate were compared between groups A and B.Results The intraoperative blood loss in group A was lower than that in B group,the difference was statistically significant(t=13.308,P=0.001);the first postoperative exhaust time in group A was shorter than that in B group,the difference was statistically significant(t=12.646,P=0.001);the hospital stay in group A was shorter than the B group,the difference was statistically significant(t=8.671,P=0.001);the VAS score of group A was lower than that of the B group,the difference was statistically significant(t=11.438,P=0.001).The postoperative complication rate in group A(5.26%)was significantly lower that in group B(17.54%),the difference was statistically significant(χ^(2)=4.254,P=0.039).Conclusion Laparoscopic treatment of acute appendicitis has certain advantages over small abdominal incision appendectomy.There is less intraoperative blood loss,faster postoperative recovery,fewer postoperative complications,and better applicability.It is worthy of promotion.
