2017至2019年老年患者感染主要病原菌分布及耐药性分析 被引量:7

Analysis of the distribution and drug resistance of the main pathogenic bacteria in elderly patients from 2017 to 2019

摘要 目的分析2017至2019年医院老年患者主要病原菌分布,并进行耐药性分析,探讨主要分布菌群的耐药特征。方法选取2017至2019年收治的老年患者,纳入标准为年龄60~90岁,临床确诊的感染患者;应用贝克曼库尔特公司MicroScan Walkaway 96 plus全自动细菌鉴定和药敏分析系统及配套的鉴定药敏卡片,采用WHONET5.6软件进行统计分析。结果病原菌分布革兰阴性菌占比较高,以大肠埃希菌、鲍曼不动杆菌、肺炎克雷伯菌和铜绿假单胞菌为主;感染的革兰阳性菌以肠球菌、金黄色葡萄球菌和表皮葡萄球菌为主;深部真菌感染以念珠菌属为主。结论老年患者罹患最多的是肺部感染,其次是泌尿系统感染;病原菌分布革兰阴性菌多见,部分病原菌呈现多重耐药;应加强老年患者病原菌检测和耐药性分析,加强社区健康宣教,严格无菌操作和医院内环境消毒,减少院内交叉感染。 Objective To investigate the distribution of main pathogenic bacteria and drug resistance characteristics in elderly patients in the Third Affiliated Hospital of Henan University of Science and Technology from 2017 to 2019.Methods The elderly patients with age being 60~90 years who were admitted and treated in our hospital from 2017 to 2019 were enrolled in the study.The bacterial identification and drug sensitivity analysis were perfomed and statistically analyzed.Results The distribution of pathogenic bacteria was dominant by Gram-negative bacteria,which were mianly escherichia coli,acinetobacter baumannii,and pneumonia klebsiella and pseudomonas aeruginosa.And the infected Gram-positive bacteria were mainly enterococcus,staphylococcus aureus,and staphylococcus epidermidis,moreover,the deep fungal infections were mainly Candida.Conclusion The elderly patients mainly suffer from pulmonary infections,followed by urinary system infections,and Gram-negative bacteria are more common in pathogens,and some pathogens are multi-drug resistant,therefor,it is necessary to strengthen pathogen detection and drug resistance analysis in elderly patients,and improve community health education,and to take strict aseptic operation and disinfection,so as to decrease the cross-infection within hospital.
