摘要
目的比较经皮侧路椎间孔镜与后路椎板开窗术治疗腰椎间盘突出症(LDH)的临床效果。方法选取2019年1月—2020年1月江西省乐平市中医医院骨科收治的60例LDH患者作为研究对象,按照随机数字表法将其分为对照组(30例)与研究组(30例)。对照组患者采用后路椎板开窗术,研究组患者采用经皮侧路椎间孔镜。比较两组患者的各项手术指标、术后并发症总发生率及手术效果。比较两组患者手术前后的疼痛情况和腰椎功能障碍情况。结果研究组患者的切口长度、手术用时、术后住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);研究组患者的术后并发症总发生率低于对照组,差异有统计学意义(P<0.05);两组患者的手术优良率比较,差异无统计学意义(P>0.05);两组患者手术前后的疼痛视觉模拟评分(VAS)与Oswestry功能障碍指数(ODI)评分比较,差异无统计学意义(P>0.05);两组患者术后1个月的VAS及ODI评分低于术前,差异有统计学意义(P<0.05)。结论后路椎板开窗术与经皮侧路椎间孔镜在LDH患者中具有相近的手术疗效,但后者对患者的创伤更小,且术后并发症更少,推荐作为首选。
Objective To compare the clinical effect of percutaneous lateral foraminal endoscopy and posterior fenestration in the treatment of lumbar disc herniation(LDH).Methods From January 2019 to January 2020,60 patients with LDH admitted to the Department of Orthopedics,Leping Hospital of Traditional Chinese Medicine,Jiangxi Province were selected as the research objects.According to the random number table method,they were divided into control group(30 cases)and study group(30 cases).Patients in the control group underwent posterior fenestration,and patients in the study group underwent percutaneous lateral foraminal endoscope.The surgical indexes,the total incidence of postoperative complications and the surgical results were compared between the two groups.The pain and lumbar dysfunction before and after surgery were compared between the two groups.Results The incision length,operation time and postoperative hospital stay in the study group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,with statistically significant differences(P<0.05).The total incidence of postoperative complications in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the rate of the excellent and good of operation between the two groups(P>0.05).There was no significant difference in pain visual analogue scale(VAS)and Oswestry disability index(ODI)scores before and after surgery between the two groups(P>0.05).The VAS and ODI scores at one month after surgery in the two groups were lower than those before surgery,with statistically significant differences(P<0.05).Conclusion Posterior lamina fenestration and percutaneous lateral foraminal endoscopy have similar surgical effects in patients with LDH,but the latter has less trauma to patients and fewer postoperative complications,it is recommended as the first choice.
