摘要
目的:探讨不同麻醉方式在腰椎间盘突出手术中的应用效果。方法:选取2021年1—12月于临沂市第四人民医院行手术治疗的腰椎间盘突出症患者90例作为研究对象,随机分为局麻组(实施局部麻醉)与硬膜外麻醉组(实施硬膜外麻醉),各45例。比较两组术后麻醉不良反应、疼痛程度、腰椎功能。结果:两组术后麻醉不良反应总发生率比较,差异无统计学意义(P>0.05)。术前、术后1周及术后6个月,两组视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05);术中、术后1周及术后6个月,两组VAS评分低于术前,且硬膜外麻醉组术中VAS评分低于局麻组(P<0.05)。术前、术后6个月,两组腰椎功能障碍指数(ODI)评分比较,差异无统计学意义(P>0.05);术后6个月,两组ODI评分均低于术前(P<0.05)。结论:局部麻醉及硬膜外麻醉应用于腰椎间盘突出症患者手术治疗中均具备良好的麻醉安全性,且不会影响术后疗效,但硬膜外麻醉的术中镇痛效果更佳,临床可根据患者具体情况选择麻醉方式。
Objective:To investigate the effect of different anesthesia methods in lumbar disc herniation surgery.Methods:A total of ninety patients with lumbar disc herniation who underwent surgical treatment in Linyi Fourth People's Hospital from January to December 2021 were selected as the study objects,and were randomly divided into local anesthesia group(local anesthesia)and epidural anesthesia group(epidural anesthesia),forty-five cases in each group.The side effects of anesthesia,pain degree and lumbar function were compared between the two groups.Results:There was no significant difference in the total incidence of anesthesia adverse reactions between the two groups(P>0.05).There was no significant difference in visual analogue scale(VAS)scores between the two groups before surgery,1 week and 6 months after surgery(P>0.05);during the operation,1 week and 6 months after the operation,the VAS score of the two groups was lower than that before the operation,and the VAS score of the epidural anesthesia group was lower than that of the local anesthesia group(P<0.05).There was no significant difference in the score of lumbar dysfunction index(ODI)between the two groups before surgery and 6 months after surgery(P>0.05);at 6 months after operation,ODI scores in both groups were lower than those before operation(P<0.05).Conclusion:Both local anesthesia and epidural anesthesia have good anesthetic safety in the surgical treatment of patients with lumbar disc herniation,and will not affect the postoperative efficacy.However,epidural anesthesia has better intraoperative analgesic effect,and the anesthesia method can be selected according to the specific conditions of patients.
