二型糖尿病与牙周炎微生物组的双向促进因素 被引量:8

摘要 牙周炎(Periodontitis,PD)是由菌斑微生物引起的牙周支持组织慢性感染性疾病,可以导致牙周支持组织的持续低度炎症和破坏。二型糖尿病(Type 2 diabetes,T2D)主要是碳水化合物代谢失调,以胰岛素分泌缺陷或胰岛素作用受损导致的高血糖为特征。糖尿病和牙周炎均是影响全民健康的重要慢性疾病。二者的相互促进关系和炎性学说机制一直是口腔医学研究的热点,但具体机制并未得到很好阐释。普遍认为微生物组在牙周炎、糖尿病的触发和恶化进程中对免疫系统等的调控发挥重要作用。本文综述了:(1)牙周炎微生物组对PD和T2D的双向促进作用;(2)固有和适应性免疫细胞群、细胞因子、脂肪因子、骨代谢和糖脂代谢等多重因素发挥的作用和可能的机制。深入理解微生物和宿主之间的关系对阐明牙周炎与糖尿病双向促进关系具有重要意义,有利于牙周炎相关糖尿病的病因探索和防治。 Periodontitis(PD)is a chronic infectious disease of periodontal support tissue caused by plaque microorganisms,which can lead to continuous low degree of inflammation and destruction of periodontal support tissue.Type 2 diabetes(T2D)is mainly a disorder of carbohydrate metabolism,characterized by hyperglycemia caused by defective insulin secretion or impaired insulin action.Diabetes mellitus and periodontitis are important chronic diseases affecting national health.The mutual promotion between the two and the mechanism of inflammatory theory have always been the focus of stomatology research,but the specific mechanism has not been well explained.It is widely believed that microbiome plays an important role in the regulation of immune system in the triggering and worsening process of periodontitis and diabetes.In this review,we discussed:(1)the bidirectional promoting effect of periodontitis microbiome on PD and T2D;(2)The roles and possible mechanisms of multiple factors such as innate and adaptive immune cell populations,cytokines,adipokines,bone metabolism and glucose and lipid metabolism.An in-depth understanding of the relationship between microorganism and host is of great significance for clarifying the bidirectional promoting relationship between periodontitis and diabetes,which is conducive to the etiology exploration and prevention of periodontitis-related diabetes.
