摘要
目的了解丽水地区人群M蛋白血症的临床特征。方法回顾性分析2017年1月—2020年5月M蛋白血症患者236例,分析M蛋白类型、免疫球蛋白量、轻链含量,并分析临床诊断病种分布;另选30例健康体检者检测其免疫球蛋白量和轻链含量与M蛋白血症患者作对照分析。结果多发性骨髓瘤(MM)138例、意义未明的单克隆免疫球蛋白血症(MGUS)39例、华氏巨球蛋白血症(WM)4例、恶性淋巴瘤9例、肾病等46例。M蛋白分型:IgG型124例占52.5%,IgA型70例占29.7%,IgM型20例占8.5%,轻链型21例占8.9%,双克隆型1例占0.4%,M蛋白对应的免疫球蛋白与正常体检比较差异有统计学意义,均有不同程度的提高,其他类型的免疫球蛋白受到抑制。结论临床医生有必要提高对M蛋白血症的认识,M蛋白可在多种恶性疾病患者血清中检出,一旦发现M蛋白,还要进一步行骨髓细胞学、骨髓病理等检查,对疾病进行确诊,决定是否治疗。
Objective To understand the clinical characteristics of M-proteinemia in Lishui area.Methods A total of 236 patients with M-proteinemia from January 2017 to May 2020 were analyzed retrospectively.The type of M-protein,the amount of immunoglobulin and the content of light chain were analyzed,and the distribution of clinical diagnosis diseases were also analyzed.Another 30 healthy people were selected to test the amount of immunoglobulin and light chain,and the patients with M-albuminemia were compared.Results A total of 138 cases were multiple myeloma(MM),39 cases were monoclonal immunoglobulinemia(MGUS),4 cases were megaglobulinemia of fahrenheit(WM),9 cases were malignant lymphoma,46 cases were nephropathy,etc.M-protein typing:124 cases of IgG type accounted for 52.5%,70 cases of IgA type accounted for 29.7%,20 cases of IgM type accounted for 8.5%,21 cases of light chain type accounted for 8.9%,1 case of Biclonal type accounted for 0.4%.The immunoglobulin corresponding to M-protein was significantly different from that of normal physical examination with different degrees of improvement.Other types of immunoglobulins were inhibited.Conclusion It is necessary for clinicians to improve their understanding of M-proteinemia.M-protein can be detected in the serum of patients with various malignant diseases.Once M-protein is found,bone marrow cytology,bone marrow pathology and other examinations should be further carried out to confirm the disease and decide whether to treat it or not.
