摘要
【目的】探究抑郁症患者血清微小RNA-381-3p(miR-381-3p)表达水平及其临床意义。【方法】选取2019年1月至2020年12月本院收治的104例抑郁症患者为抑郁症组,另纳入同期104例体检健康者为健康组。比较抑郁症组、健康组一般资料、血清miR-381-3p、脑源性神经营养因子(BDNF)水平;利用24项汉密尔顿抑郁量表(HAMD-24)评估患者抑郁程度,将患者分为重度组(n=31)、中度组(n=33)、轻度组(n=40),比较不同抑郁程度患者血清miR-381-3p、BDNF水平;Pearson法分析抑郁症患者血清miR-381-3p、BDNF水平与HAMD-24评分的相关性;Logistic回归分析抑郁症发生的影响因素。【结果】抑郁症组患者饮酒占比、受教育年限、男/女比例、年龄、吸烟占比、体重指数(BMI)与健康组无明显差异(t/χ^(2)=0.515、1.050、0.942、0.618、0.481、1.000,P=0.473、0.295、0.332、0.537、0.488、0.318),血清miR-381-3p表达水平高于健康组(t=13.950,P<0.05),BDNF水平低于健康组(t=9.591,P<0.05);血清miR-381-3p表达水平随抑郁症患者病情加重呈升高趋势(F=9.726,P<0.05),血清BDNF水平随病情加重而呈降低趋势(F=28.543,P<0.05);抑郁症患者血清miR-381-3p水平与HAMD-24评分呈正相关(r=0.587,P<0.05),与血清BDNF水平呈负相关(r=-0.418,P<0.05);抑郁症患者血清BDNF水平与HAMD-24评分呈负相关(r=-0.576,P<0.05);miR-381-3p是抑郁症发生的危险因素(95%CI=1.785~4.663,P<0.05),BDNF是抑郁症发生的保护因素(95%CI=0.480~0.716,P<0.05)。【结论】抑郁症患者血清miR-381-3p表达水平较高,与BDNF、HAMD-24评分显著相关,其有望成为评估抑郁程度的潜在标志物。
【Objective】To explore the expression level of serum microRNA-381-3p(miR-381-3p)and its clinical significance in patients with depression.【Methods】A total of 104 patients with depression admitted to our hospital from January 2019 to December 2020 were selected as the depression group,and 104 patients with healthy physical examination during the same period were included as the healthy group.The general data,serum miR-381-3p,brain-derived neurotrophic factor(BDNF)levels in depression group and healthy group were compared;the degree of depression in patients was assessed with 24 Hamilton Depression Scale(HAMD-24),and the patients were divided into the severe group(n=31),the moderate group(n=33)and the mild group(n=40),and the levels of serum miR-381-3p and BDNF in patients with different degrees of depression were compared;Pearson method was used to analyze the correlation between serum miR-381-3p and BDNF levels and HAMD-24 score in patients with depression;Logistic regression was used to analyze the influencing factors of depression.【Results】There was no significant difference in the proportion of alcohol consumption,years of education,male/female ratio,age,proportion of smoking,body mass index(BMI)between the depression group and the healthy group(t/χ^(2)=0.515,1.050,0.942,0.618,0.481,1.000,P=0.473,0.295,0.332,0.537,0.488,0.318),the expression level of serum miR-381-3p in the depression group was higher than that in the healthy group(t=13.950,P<0.05),and the level of BDNF was lower than that in the healthy group(t=9.591,P<0.05);the expression level of serum miR-381-3p showed an increasing trend with the aggravation of depression in patients(F=9.726,P<0.05),and the level of serum BDNF showed a decreasing trend with the aggravation of the disease(F=28.543,P<0.05);the level of serum miR-381-3p in patients with depression was positively correlated with HAMD-24 score(r=0.587,P<0.05),and negatively correlated with the level of serum BDNF(r=-0.418,P<0.05);the level of serum BDNF in patients with depression
