摘要
目的基于隐结构模型探讨糖尿病的证候及辨证规律,为糖尿病的临床辨证规律提供参考。方法检索中国期刊全文数据库(CNKI)、中文科技期刊数据库(VIP)、万方数据知识服务平台等数据库内1990年—2018年11月的名老中医治疗糖尿病的病案及临证经验,经筛选后,用Lantern 5.0软件构建糖尿病症状的隐结构模型,结合互信息、信息覆盖率、条件概率、隐类概率等数据,人工进行模型诠释,最后将隐类进行综合聚类,提取证候信息并分析糖尿病的证候分布及辨证规律。结果共纳入163例糖尿病医案,以症状、舌、脉共108个显变量构建症状隐结构模型,得到隐变量8个,隐类16个,3个综合聚类模型。结论糖尿病的病理因素多为阴虚、燥热、痰(湿)热、肾阳虚,主要证候有痰热互结证、胃热炽盛证、肾阳衰微证。
Objective To explore the syndrome and syndrome differentiation rules of diabetes based on the implicit structure model, and provide reference for clinical syndrome differentiation and treatment of diabetes. Methods We searched the articles about traditional Chinese medicine physicians’ experience on traditional Chinese medicine treatment of diabetes from Chinese journal full-text database(CNKI), Chinese journal service platform(VIP), Wanfang data knowledge service platform and other database built-in library from 1900 to November 2018. After screening, Lantern 5.0 software was used to construct the implicit structure model of diabetes symptoms, combined with mutual information, information coverage, conditional probability, implicit probability and other data, we manually interpreted the model, and finally clustered the hidden classes, extracted the syndrome information and analyzed the distribution of syndromes and syndrome differentiation rules of diabetes. Results A total of 163 cases of diabetes were included. The symptom implicit structure model was constructed with 108 variables of symptoms, tongue and pulse. Eight hidden variables, 16 hidden classes and three comprehensive clustering models were obtained. Conclusion The pathological factors of diabetes are mostly Yin deficiency, dry heat, phlegm(dampness) heat, and kidney Yang deficiency. The main syndromes include phlegm(dampness) heat syndrome, stomach heat stagnation syndrome, and kidney Yang deficiency.
