摘要
目的探讨丁苯酞软胶囊对症状性慢性颈动脉闭塞患者脑血流灌注、认知功能及血清基质金属蛋白酶-9(MMP-9)的影响。方法选取2015年2月—2020年2月在太原市中心医院神经内科住院的经全脑血管造影诊断为症状性慢性颈动脉闭塞患者56例,采用随机数字法分为对照组和观察组各28例,2组均接受常规治疗,观察组在常规治疗基础上联合丁苯酞软胶囊治疗,疗程3个月。观察2组患者治疗后脑血流灌注参数、简易智能精神状态检查量表(MMSE)评分及血清MMP-9水平变化。结果治疗后观察组相对脑血流量(rCBF)及MMSE评分明显高于治疗前及对照组(P均<0.05),相对平均通过时间(rMTT)、相对达峰时间(rTTP)及血清MMP-9水平均明显低于治疗前及对照组(P均<0.05)。观察组治疗后rCBF增加值与MMSE评分呈正相关关系(r=-0.296,P=0.043),rCBF增加值与血清MMP-9降低值呈负相关关系(r=-0.464,P=0.013),血清MMP-9降低值与MMSE评分增加值呈负相关关系(r=-0.855,P<0.001)。结论丁苯酞软胶囊可提高症状性慢性颈内动脉闭塞患者缺血区脑血流灌注水平,抑制MPP-9生成,减轻炎症反应,改善其认知功能。
Objective It is to explore the effects of Butylphthalide soft capsules on cerebral blood flow perfusion,cognitive function and serum matrix metalloproteinase-9(MMP-9)in patients with chronic symptomatic carotid occlusion.Methods Fifty-six patients with chronic symptomatic carotid artery occlusion diagnosed by whole cerebral angiography in the Department of Neurology of Taiyuan Central Hospital from February 2015 to February 2020 were selected and divided into control group and observation group by random number method,with 28 cases in each group.Both groups received conventional treatment,and the observation group was treated with Butylphthalide soft capsule on the basis of conventional treatment,and the course of treatment was 3 months.The changes in cerebral blood perfusion parameters,MMSE scores and serum MMP-9 levels after treatment were observed in the two groups.Results After treatment,the relative cerebral blood flow(rCBF)and MMSE scores of the observation group were significantly higher than those before treatment and in the control group(all P<0.05),the relative mean transit time(rMTT),relative time to peak(rTTP)and serum MMP-9 levels were significantly higher were significantly lower than those before treatment and in the control group(all P<0.05).The increase value of rCBF after treatment was positively correlated with the MMSE score in the observation group(r=-0.296,P=0.043),the increase value of rCBF was negatively correlated with the decrease value of serum MMP-9(r=-0.464,P=0.013),and the decreased value of serum MMP-9 was negatively correlated with the increased value of MMSE score(r=-0.855,P<0.001).Conclusion Butylphthalide soft capsule can increase the level of cerebral blood flow perfusion in the ischemic zone,inhibit the production of MPP-9,alleviate inflammatory reaction and improve the cognitive function in patients with chronic symptomatic internal carotid artery occlusion.
