右美托咪定联合丙泊酚和舒芬太尼在电子支气管镜检查中的临床有效性分析 被引量:3

摘要 目的探讨右美托咪定联合丙泊酚与舒芬太尼用于电子支气管镜检查实践中的有效性。方法68例行支气管镜检查的患者,根据麻醉方法不同分为DPS组与PS组,各34例。DPS组采用右美托咪定联合丙泊酚、舒芬太尼麻醉,PS组患者采用泊酚与舒芬太尼麻醉。比较两组警觉/镇静评分(OAA/S)、血流动力学指标检测情况、手术时间、不良反应发生情况。结果入室时(T0)、麻醉诱导后(T1),两组平均动脉压(MAP)、心率(HR)、血氧饱和度(SPO2)、水平及OAA/S评分比较,差异无统计学意义(P>0.05);支镜通过声门时(T2)、支镜抵达隆突时(T3)、检查完成时(T4),DPS组SPO2、OAA/S评分高于PS组,HR、MAP低于PS组,差异均有统计学意义(P<0.05)。DPS组操作时间、麻醉时间分别为(18.6±7.8)、(26.4±7.8)min,与PS组的(19.2±6.6)、(24.8±6.7)min比较,差异无统计学意义(P>0.05);DPS组患者苏醒时间为(2.9±1.0)min,长于PS组的(1.9±0.7)min,差异有统计学意义(P<0.05)。DPS组不良反应发生率为52.9%,低于PD组的82.4%,差异有统计学意义(P<0.05)。结论支气管镜检查时采用右美托咪定联合丙泊酚和舒芬太尼进行复合麻醉,能取得较好的麻醉效果,能维持血流动力学的相对稳定性,值得推广。 Objective To discuss the effectiveness of dexmedetomidine combined with propofol and sufentanil in electronic bronchoscopy.Methods A total of 68 patients with electronic bronchoscopy were divided into DPS group and PS group according to different anesthesia methods,with 34 cases in each group.DPS group received dexmedetomidine combined with propofol and sufentanil for anesthesia,and PS group received propofol and sufentanil for anesthesia.The observer’s assessment of alertness/sedation(OAA/S),detection of hemodynamic indexes,operation time and occurrence of adverse reactions were compared between the two groups.Results When entering the room(T0)and after induction of anesthesia(T1),there was no statistically significant difference in mean arterial pressure(MAP),heart rate(HR),blood oxygen saturation(SPO2)level and OAA/S score between the two groups(P>0.05).When the bronchoscope passed the glottis(T2),when the bronchoscope reached the carina(T3),and when the examination was completed(T4),the SPO2 and OAA/S score of DPS group were higher than those of PS group,and HR and MAP were lower than those of PS group.All the difference was statistically significant(P<0.05).The operation time and anesthesia time of DPS group were(18.6±7.8)and(26.4±7.8)min,which had no statistically significant difference compared with(19.2±6.6)and(24.8±6.7)min of PS group(P>0.05).The wake-up time(2.9±1.0)min of DPS group was longer than(1.9±0.7)min of PS group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions of DPS group was 52.9%,which was lower than 82.4% of PD group,and the difference was statistically significant(P<0.05).Conclusion During electronic bronchoscopy,dexmedetomidine combined with propofol and sufentanil for anesthesia can achieve good anesthetic effect and maintain the relative stability of hemodynamics,which is worthy of promotion.
