电针联合系统性康复训练对前交叉韧带损伤术后患者等速肌力、关节活动度及机体功能的影响 被引量:9

Effect of Electroacupuncture Plus Systematic Rehabilitation Training on Isokinetic Muscle Strength,Joint Mobility and Body Function in Patients After Surgery for Anterior Cruciate Ligament Injury

摘要 目的观察电针联合系统性康复训练对前交叉韧带损伤(ACL)术后患者等速肌力、关节活动度及机体功能的影响。方法选取ACL患者92例,按照简单随机数字表法分为研究组和对照组,每组46例。两组均行关节镜下膝前交叉韧带重建术,对照组术后采取系统康复训练,研究组术后在对照组基础上联合电针治疗。比较两组治疗优良率、治疗前后机体功能康复评分(Lysholm)、等速肌力[屈膝相对峰力矩(PT/BW)、腘绳肌与股四头肌肌力比值(H/Q)、屈和伸膝峰力矩值(PT)]、疼痛视觉模拟评分法(VAS)评分及Lachman试验、膝关节活动度、患膝肿胀程度。结果治疗1个月、3个月后,研究组Lysholm评分中下蹲、爬楼梯、肿胀、疼痛、关节不稳、交锁、负重、跛行评分均高于对照组(P<0.05),研究组屈膝PT/BW、H/Q、伸膝PT、屈膝PT均大于对照组(P<0.05),研究组VAS评分和Lachman试验均优于对照组(P<0.05)。治疗后,研究组治疗优良率高于对照组,差异有统计学意义(P<0.05)。治疗后,研究组膝关节活动度大于对照组(P<0.05),患膝肿胀程度小于对照组(P<0.05)。结论电针联合系统性康复训练能有效减轻ACL术后关节疼痛,缓解关节肿胀程度,提高等速肌力、膝关节稳定性和活动度,进一步改善膝关节功能。 Objective To investigate the effect of electroacupuncture plus systematic rehabilitation training on isokinetic muscle strength,joint mobility and body function in patients after surgery for an anterior cruciate ligament(ACL)injury.Method Ninety-two patients with ACL injury were allocated,using a random number table,to a study group and a control group,with 46 cases in each group.Both groups underwent arthroscopic anterior cruciate ligament reconstruction.After surgery,the control group received systematic rehabilitation training and the study group,electroacupuncture in addition.The excellent and good rates,and pre-treatment and post-treatment physical function rehabilitation scores(Lysholm),isokinetic muscle strengths[knee flexion peak torque/body weight(PT/BW),hamstring-to-quadriceps(H/Q)strength ratio and knee flexion and extension peak torque(PT)],visual analog scale(VAS)scores,Lachman tests,knee joint mobility and affected knee swelling severities were compared between the groups.Result After one and three months of treatment,the Lysholm squatting,stair-climbing,swelling,pain,joint instability,interlocking,weight-bearing and lameness scores were higher(P<0.05),knee flexion PT/BW,H/Qstrength ratio,knee extension PT and knee flexion PT were larger(P<0.05)and the VAS score and Lachman test were better(P<0.05)in the study group than in the control group.After treatment,the excellent and good rate was higher in the study group than in the control group with a statistically significant difference(P<0.05).After treatment,knee activity was larger(P<0.05)and affected knee swelling severity was smaller(P<0.05)in the study group than in the control group.Conclusion Electroacupuncture plus systematic rehabilitation training can effectively relieve joint pain,reduce joint swelling and increase isokinetic muscle strength and knee stability and activity after surgery for an ACL injury to further improve knee function.
