摘要
目的探讨社区老年人的非酒精性脂肪肝(NAFLD)患病率及其危险因素。方法收集2018年广州市某社区健康体检老年人资料1 912例,根据肝脏B超将其分为NAFLD组(n=406)与非NAFLD组(n=1 506),采用Logistic回归分析相关危险因素。结果 NAFLD患病率为21.23%(406/1 912)。男性患病率19.58%(158/807)与女性患病率22.44%(248/1 105)比较差异无统计学意义(P>0.05)。单因素分析显示,NAFLD组与非NAFLD组患者体重、空腹血糖、体重指数(BMI)、甘油三酯、高密度脂蛋白胆固醇(HDL-C)、尿酸、丙氨酸氨基转移酶、天门冬氨酸氨基转移酶、白细胞以及高血压、糖尿病比例比较差异有统计学意义(P<0.05)。多因素分析显示,空腹血糖、BMI、甘油三酯、丙氨酸氨基转移酶、高血压与NAFLD呈正相关,差异均有统计学意义(P<0.05)。结论广州某社区五分之一老年人患有非酒精性脂肪肝,高血糖、高BMI和高血脂是其主要的危险因素。
Objective To investigate the prevalence of risk factors of nonalcoholic fatty liver disease(NAFLD)in the elderly in the community in Guangzhou. Methods The data of physical examination of 1 912 elderly people in 2018 were collected in a community of Guangzhou. They were collected and divided into NAFLD group(n=406)and non-NAFLD group(n=1 506)according to liver ultrasound and the relevant risk factors were analyzed by using Logistic regression.Results The prevalence of NAFLD was 21.23%(406/1 912). The prevalence was 19.58%(158/807) in males and22.44%(248/1 105)in females(P>0.05). Univariate analysis showed that body weight,fasting blood glucose,body mass index(BMI),triglyceride,high-density lipoprotein-cholesterol(HDL-C),uric acid,serum alanine transaminase,aspartate transaminase,white blood cells,proportion of hypertension and diabetes in NAFLD group were significantly different from those in non-NAFLD group(P<0.05). Logistic regression analysis showed that fasting plasma glucose,BMI,triglyceride,serum alanine transaminase and hypertension were positively correlated with NAFLD in the elderly(P<0.05). Conclusions One in five elderly people in Guangzhou community suffers from NAFLD. High blood glucose,high BMI and high blood lipid were the main risk factors for the NAFLD of community elderly.
