摘要
目的探讨血栓分子标志物对癌症患者发生血栓的早期诊断价值。方法选取385例住院治疗的癌症患者作为研究对象,按照肿瘤类型分为肺癌组(155例)、乳腺癌组(102例)、胃癌组(67例)、结直肠癌组(61例),选择同期健康体检者50名作为对照组。收集每位癌症患者化疗前和化疗1个周期后的血浆样本,检测血栓分子标志物血栓调节蛋白(TM)、纤溶酶-纤溶酶抑制剂复合物(PIC)、凝血酶-抗凝血酶复合物(TAT)、组织型纤溶酶原激活物-纤溶酶原激活物抑制剂复合物(t PAI-C),对比各组化疗前血栓分子标志物水平,分析癌症患者血栓分子标志物与D-二聚体的相关性,对比化疗前后血栓分子标志物水平。结果癌症各组的化疗前血栓分子标志物TM、PIC、TAT、tPAI-C水平均显著高于对照组(P<0.05);血栓分子标志物PIC、TAT与D-二聚体存在明显的正相关关系(r=0.683,P=0.000;r=0.706,P=0.000);血栓分子标志物TM、tPAI-C与D-二聚体无明显相关关系(r=0.04,P=0.627;r=0.07,P=0.690);化疗后血栓分子标志物PIC、TAT水平较化疗前降低(P<0.05);化疗前后血栓分子标志物TM、tPAIC水平无明显变化(P>0.05)。结论癌症患者的血栓分子标志物TM、PIC、TAT、tPAI-C表达显著升高,化疗后PIC、TAT水平较化疗前降低,PIC、TAT与D-二聚体存在明显的正相关关系,对癌症患者血栓的诊断有一定价值。
Objective To investigate the effect of thrombus molecular markers on early diagnosis of thrombosis in cancer patients.Methods 385 hospitalized cancer patients were divided into lung cancer group(155 cases),breast cancer group(102 cases),gastric cancer group(67 cases)and colorectal cancer group(61 cases)according to the tumor type.50 healthy subjects who had physical examination at the same time were selected as the control group.Plasma samples of each cancer patient before chemotherapy and after one cycle of chemotherapy were collected to detect the levels of TM,PIC,Tat and tpai-c.The levels of thrombus molecular markers in cancer patients and the control group before chemotherapy were compared.The correlation between the levels of thrombus molecular markers and D-dimer in cancer patients was analyzed,and the differences of thrombus molecular markers before and after chemotherapy were compared.Results The levels of TM,PIC,Tat and tpai-c in cancer groups before chemotherapy were significantly higher than those in control group(P<0.05);there was a significant positive correlation between thrombus molecular markers PIC,TAT and D-Dimer.There was no significant correlation between TM,tPAI-C and D-Dimer.After chemotherapy,the levels of PIC and TAT were lower than those before chemotherapy(P<0.05),while the levels of TM and tPAI-C had no significant changes before and after chemotherapy(P>0.05).Conclusion The expression of TM,PIC,TAT and tPAI-C in cancer patients is significantly increased,and the levels of PIC and TAT after chemotherapy are lower than those before chemotherapy.There is a positive correlation between PIC,TAT and D-dimer,which has a certain value in the diagnosis of thrombosis in cancer patients.
