摘要
目的:观察身痛逐瘀汤加味联合针刺治疗急性腰椎间盘突出症的疗效。方法:将2018年3月-2020年2月广州市白云区石井人民医院收治的急性腰椎间盘突出症患者86例,随机分为两组,每组各43例。对照组予以西医常规治疗,治疗组予以西医常规治疗联合身痛逐瘀汤加味联合针刺治疗,疗程为2周。通过中医证候疗效、中医证候评分、视觉模拟评分法(Visual Analogue Scale,VAS)、Oswestry功能障碍指数(Oswestry Disability Index,ODI)、改良日本骨科学会下腰痛评分量表(Japanese Orthopedic Association Lao Back Pain Score,M-JOA)、症状缓解时间、血清炎症因子[肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、白细胞介素(Interleukin,IL)-1β、IL-6]、氧化应激[超氧化物歧化酶(Superoxide Dismutase,SOD)、丙二醛(Malondialdehyde,MDA)]评价疗效。结果:治疗组的总有效率高于对照组,差异具有统计学意义(P<0.05);两组治疗前中医证候积分、VAS评分、ODI评分、M-JOA评分以及血清TNF-α、IL-1β、IL-6、SOD水平的差异均无统计意义(P>0.05),治疗后两组上述指标均较治疗前显著降低(P<0.05),且治疗后治疗组上述指标改善程度显著优于对照组,差异均具有统计学意义(P<0.05);两组治疗前血清SOD水平差异均无统计意义(P>0.05),治疗后两组均较治疗前显著升高(P<0.05),且治疗后治疗组改善程度显著优于对照组,差异均具有统计学意义(P<0.05);治疗后,治疗组腰部症状和下肢症状缓解时间均短于对照组,差异均具有统计学意义(P<0.05)。结论:身痛逐瘀汤加味联合针刺治疗急性腰椎间盘突出症可以缓解疼痛,改善腰椎功能,减轻炎症反应,减轻氧化应激反应。
Objective:To observe the curative effects of the Shentong Zhuyu decoction(身痛逐瘀汤)plus acupuncture on acute lumbar disc herniation.Methods:86 cases were randomly divided into two groups,43 cases in each group.The control group was given conventional Western medicine,and the treatment group was given Western medicine plus the Shentong Zhuyu decoction and acupuncture for 2 weeks.Results:The total efficiency in the treatment group was higher than the control group(P<0.05).The scores of TCM syndrome,VAS,ODI,and M-JOA,and the levels of TNF-α,IL-1β,and IL-6 in the groups were significantly lower than before treatment(P<0.05),and the improvement of the above indicators in the treatment group was significantly better than the control group,P<0.05.SOD in the groups were significantly higher than before(P<0.05),and the improvement in serum SOD in the treatment group was significantly better than the control,P<0.05.After treatment,the relief time of lumbar symptoms and lower limb symptoms in the treatment group was shorter than the control group,P<0.05.Conclusion:The Shentong Zhuyu decoction plus acupuncture on acute lumbar disc herniation can relieve pain,improve lumbar function,and reduce inflammatory response and oxidative stress reaction.
