大鼠腰神经根性疼痛相关行为变化与硬膜外注射白细胞介素1受体拮抗剂干预的影响 被引量:3

摘要 目的:探讨不同剂量白细胞介素1受体拮抗剂硬膜外注射对腰神经根性疼痛大鼠相关行为的影响。方法:实验于2004-05/06在兰州大学实验中心神经机能实验室及兰州大学第一医院外科实验室完成。健康成年雄性Wistar大鼠60只,将60只大鼠随机分为5组,每组12只。分别为假手术组、对照组、白细胞介素1受体拮抗剂1mg/L,50mg/L和100mg/L3个剂量组。白细胞介素1受体拮抗剂1,50,100mg/L3个剂量组,将大鼠自体尾椎间盘取出无机械压迫移植于L4~5神经根表面,通过硬膜外置管注射不同浓度的白细胞介素1受体拮抗剂。对照组以等量磷酸盐缓冲液代替,假手术组仅置管不做任何注射。使用前用磷酸盐缓冲液稀释,分别在术前1d及术后不同时间(1,4,7,11,14,21,28d)观测疼痛相关性行为指标。结果:60只大鼠均进入结果分析。①大鼠腰神经根无明显机械压迫:放置自体椎间盘可提高同侧下肢机械和热刺激痛觉敏感性。②硬膜外应用白细胞介素1受体拮抗剂能改善大鼠的疼痛相关性行为:与对照组比较,白细胞介素1受体拮抗剂1mg/L组,50mg/L组和100mg/L组在术后1,4,7d的机械刺激过敏程度明显减轻(P均=0.0001);各白细胞介素1受体拮抗剂组热痛觉敏感显著降低,在术后4,7,14d差异有极显著性(P均=0.0001)。结论:白细胞介素1在椎间盘相关性腰腿痛发病中具有与炎性疼痛相关的某种作用,白细胞介素1受体拮抗剂对自体椎间盘所致神经根炎性反应有抑制作用,白细胞介素1受体拮抗剂硬膜外应用有可能提高腰腿痛的治疗效应。 AIM:To investigate the effects of epidural injection of interleukin- 1 recep tor antagonist(IL- 1Ra) at different doses on pain- related behavior in rats w ith lumbar radiculopathy. METHODS:The experiment was conducted in the laboratory for nervous function o f experiment center in Lanzhou University and Surgical Laboratory in the First H ospital of Lanzhou University from May to June 2004.Sixty healthy adult Wistar r ats were randomly and equally divided into 5 groups,sham operation group,control group,1 mg/L IL- 1Ra group,50 mg/L IL- 1Ra group and 100 mg/L IL- 1Ra group. The intervertebral disc harvested from coccygeal vertebra of rats was applied on to the surface of L4 and L5 nerve roots without mechanical compression.IL- 1Ra diluted with phosphate buffer at different doses was injected into the cavum epi durale using an epidurally placed catheter in the IL- 1Ra groups,phosphate buff er at equivalent doses in the sham operation group,and nothing in the control gr oup.The pain- related behavior indexes were measured before operation,at 1,4,7, 11,14,21,28 days after operation. RESULTS:All the 60 rats were included in the result analysis.① Obvious incre ase in paw mechanical withdrawal threshold and thermal withdrawal latency of the operated hinder limbs occurred after application of autologous intervertebral d isc on the rat lumbar nerve roots without mechanical compression.② After IL- 1 Ra injection,pain- related behavior in rats was changed obviously.As compared w ith the control group,the mechanical withdrawal threshold in the three IL- 1Ra groups was all alleviated at postoperative 1,4 and 7 days(all P=0.000 1),and the thermal withdrawal latency was all decreased,significantly different at postope rative 4,7 and 14 days(all P=0.000 1). CONCLUSION:IL- 1 may play a certain role in the pathogenesis of lumbar disc herniation- related lumbago and leg pain.The injection of IL- 1Ra has an inhib itive effect on radiculopathy caused by application of autologous intervertebral disc.Epidural application of IL- 1Ra
